Taco Bell - double nacho $5. meal deal/ service

I stopped at Taco Bell tonight for the advertised Double Nacho. The customer in front of me was having trouble understanding the girl taking his order. He asked her to repeat herself numerous times. I was next. When I asked for the Double Nacho she mumbled something to me. I asked her to repeat herself, she mumbled again. I asked her to speak up - TWICE. It seems they didn't have them.

Why did someone put an employee on the register how clearly can not communicate with customers?
Why would they not have the advertised Double Nachos?

We visited this location at 4:45 pm on May 31.
