I went to Urgent care on December 29th. I received an x-ray that showed I had a broken foot. The following day I returned to Urgent care to have the doctor sign a form so I could get a temporary disability card. I was told that Urgent care didn't do that and I would have to see my primary. My primary doctor retired last year (Dr Jill Schmitt) and I didn't have an appointment for another doctor until April. I was told to call a phone number that could help patients and they would get me right in to a doctor that could sign my form (why I need to see a second doctor when a doctor had already looked at my x-rays and determined my foot was broken). When I called the number the lady told me she had no idea why Urgent care would tell me that because she couldn't get me in for 4 months. I find it very unresponsive to my needs that the Doctor that saw me can't sign a simple form and forces me to try to see another doctor when your system doesn't have time for me to see another doctor for 4 months. I'm 67 years old and just this little help from your doctors would be very helpful for me especially with the snow and ice now.
Desired outcome: I just want my temporary disability form sign in a reasonable fimeframe
