I had ordered glasses on the 12th of September. The glasses came in and were the wrong prescription. The doctor checked and they were way off with a stigmatism. He sent them back to your lab and called them about the problem. The glasses came back again with the exact same problem. they were sent back again. This time they were closer off a quarter but still had stigmatism which I do not have. He said they will not listen to him, and this is probably the best their lab can do. They don't cut the lenses the way other labs do, with eye distance from nose, and other measurements that became a problem. I told him refund on the glasses. I called up Davis vision and was told that out of network covers only $35 for a single vision lens. I wouldn't have this problem if your lab cut the glasses right in the first place. So I have insurance I can't use because the incompetence of your lab. How can I get this issue rectified. I am not sure if I should contact the state about this issue. This appears to be a problem for your company. I am not sure if this is an attorney general's office complaint or contact the City of Chicago about their insurance carrier, or a personal attorney about getting the benefits I deserve. Thank you for any and all consideration you can give to this matter.
Names of doctor and my information can be obtained by a response.
Desired outcome: Use an out of network provider to cut lens due to the incompetence of your lab Davis Vision.
Update: After numerous calls to Davis vision and talked to supervisor Debora T************ who reinstated benefits, along with approving another exam (up to $100 reimbursement credit )to go to another Davis vision provider. After some research I found that Vision works has its own lab to cut glasses. They do not use Davis labs. The supervisor called Vision works and determined their exam was $84 and stated they will cover the exam upon receipt for that exam. I had the exam and purchased glasses on 7 November. The exam was $84 and I submitted the receipt to Supervisor Debora. She acknowledged receipt and told me a DSA check will be sent to me in 4 weeks. After 5 weeks I called and asked if the check was processed and was told that vision works does a part of their exam that is not covered so they can't cover the exam. So one part of a 20 point of exam not covered so they won't cover any of it, after saying that the only restriction was that they would cover anything under $100. 4 months and counting. Now won't return emails or calls. I am filing a complaint with the Illinois Attorney General after Christmas. Terrible company.
