On November 23 2020
I had an appointment at Family Planning Associates Medical Group. Stephanie the supervisor said I can't be seen because I have my two year old son. I told Stephanie that I got rescheduled and that on November 20th which is on a friday that I explained that I have to bring my baby because I have no one to watch him the lady said ok.
Stephanie said that it's the policy that no one can be with me and I told stephanie well I was told to reschedule again and I Stephanie I just want to get this done already and that I been having issues since the beginning of the month and that I can't keep getting rescheduled I have to be seen. Stephanie said I don't know what to tell you this is not my problem. Stephanie said it's going to be hard to do an ultrasound with the baby in the room and I told Stephanie that he has a car seat and I can strap him in then Stephanie said that she talked to the doctor and the doctor said he won't see me because I have the baby with me. Stephanie said she feels unsafe with the baby being in waiting room and that I have to find a setter to be seen by the doctor. I told Stephanie I want to talk to the doctor Stephanie said no you can't. I told Stephanie that I been having problems with this office since the begging of the month. Stephanie was laughing and smiling saying well that's not my problem and I don't know what to tell you it's really not my problem but I can reschedule you. I told her that I'm just going to file a complaint on her and this office because of all the rescheduling. Stephanie said do what you have to do and I told her I will.
Stephanie was laughing and smiling thru the whole conversation. I was told by Stephanie a couple of time this is not her problem and not her issues to be dealing with and then Stephanie said I don't know what else's to tell you but reschedule you.

I told Stephanie I don't want to talk to her I want to talk to someone else's. Stephanie said It's only her in the office so you can speak to me and I kept telling Stephanie that I don't want to talk to her Stephanie said well I'm here and you don't have to be here. Stephanie kept saying it's her office and that if there's going to problems go file a complaint while laughing and smiling. The only thing I kept getting from Stephanie was
I don't know what to tell you.
It's not my problem or issue.
I can reschedule you if you want.
I really don't know what to tell.
Stephanie was laughing the whole time in the conversation.
I was told by Stephanie repeatedly that it's her office and it's up to her for me to see the doctor.

On November 24 2020
I filled 3 complaints to Care1st.
I filled a complaint to Arizona medical board.
I filled a complaint to Arizona health department.

The person I filled a complaint on is Stephanie she is the supervisor at Family Planning Associates Medical group.
she is not the doctor she in an employee

November 24 2020 I told Stephanie I filled a complaint on her. Stephanie said she feels unsafe and threaten and that I will not be seen on December 2nd by the Ever since I told Stephanie I have filled complaint on her she's been saying she feels unsafe. I have the right to complain and I have the right to file a complaint on her.

On November 25 I got a phone call by the clinic and I was told I will not be seen because according to Stephanie she said I was yelling and threaten her and the staff and the doctor. I told the person that she is lying I told the staff I never talked to the staff or the doctor. I told the staff I talked to her yesterday and that she said feels threatened because I told her I filled a complaint on her. I told the worker I never threaten her or any one there all I told Stephanie was I'm filing a complaint on you. Stephanie won't let me talk to the doctor or anyone else in the clinic. After the conversation the staff said I can't be seen because Stephanie feels unsafe and then I got hung up on.
I never talked to the staff or talked to the doctor I only been talking to Stephanie. I keep telling Stephanie I don't want to talk to her.
