I was injured at banner thunderbird hospital on a routine echocardiogram
On May 15th in the morning I was having an eco cardiogram test and the technician twistedmy arm in a awful position for an hour. I have had terrible pain and had an MRI done two weeks ago and was told I have a tear in my rotator cuff measuring 2.5 cm and a tear in my bicep 2c long. I am in terrible pain and was told it is not surgidally fixable. All I can do is take steroid shots and will have to live with this forever.
Desired outcome: They should be repremanded and the technician should be retrained I am 84 and should not have been treated like a 24 tear old. Our bones and structures are different. I do have a copy of the MRI report if you need it.
Improper handling of medical lab samples
On July 21, 2023, my wife was informed her UDT indicated five times normal levels for Methadone. The fact is that my wife does not take opioids and has never taken any form of Methadone. It is our assertion that a mistake was made either by Dr. Dax Trujillo, MD, operating as Summit Healthcare Pain Clinic Associates. in the handling or preparation of her UDT to send to a lab, or the lab he uses to process UDT samples made a protocol error in their handling and labeling.
Requests have been made to Dr. Trujillo for all lab reports, his personal notes of all visits and those of his employees that interacted with my wife on her prior visit and the current 7.21.2023 visit.
Desired outcome: To identify the lapse in proper protocols for UDT handling and processing at either facility. Secondarily, a serious error that has now become part of my wife's medical history that should be expunged.
Surgery done
Over a year and a half ago, Dr Michael Cristiano
neurosurgeon implanted a spinal cord stimulator in my back,
unfortunately, he placed directly under my skin where it could be seen and cause incredible pain. About 6 months ago i went to him when he advised me that abbot who makes the stimulator had a new smaller one.
Two months later he advised me that there was a problem with abbot t and st Josephs hospital and would not be able to do the surgery
he referred me to pain institute of Arizona where I saw Dr Duran and am scheduled for surgery 8/4 I have been in incredible pain since his mistake when implanting the stimulator. He should not be doing this type of surgery, Barbara Spinelli

TEC AZ Emergency Physicians 70 S Val Vista Dr Gilbert AZ 85296 - FRAUD
I just received a bill in the mail from this company from 10/14/2021 for an electrocardiogram ECG - I never went to them for an ECG, yet they are claiming I own them $38.30
So, I had this done supposedly in 2021, and I am now just receiving a bill?!?!? This is either a scam or fraud and I am not sending one dime.
I am also contacting my insurance to let them know that someone in this company is filing false claims.
Desired outcome: This false claim is resolved, and I never receive a "pretend" bill for a service never provided.

In 2017 Dr Nagaiah insisted on chemotherapy administered to my father, Wallace Nelson for a diagnosis of Hodgkin Lymphoma B. Wallace Nelson never had a positive biopsy for Hodgkin Lymphoma and merely had severe osteoporosis. Wallace Nelson suffered severe cognitive and heart effects from the chemotherapy and passed away 7 months after his first treatment of a heart attack. Other than the osteoporosis, Wallace Nelson was healthy prior to the chemotherapy.

Improper billing practices
My son suffered a dislocated ankle in August of 2022 and was provided care by TEC Emergency Physicians of Arizona. My gave him all his insurance information. We never received a bill of services and neither did our insurance. However, 6 months later we received a letter from a collections agency requesting payment for services provided by TEC Emergency Physicians. I attempted to contact TEC however they forwarded me to the collection agency. So in whole, they are not billing the insurance companies at all and going directly to a collection agency and tagging on interest as well. I would like to be contacted directly by the physician group represenative to go over all services provided and have them speak with my insurance company to resolve the payment necessary. Extremely improper to utilize a collection agency as a billing service.
Doctor abandonment
Nicholas J Collins 4-17-23. Went to neurology associates mesa Higley rd. Saw Dr Andrea An. Has had grand mal seizures Not under control. Office sent us chart notes and found multiple errors and incorrect pt. Info. We advised Dr An today, she got angry, told Nick it was his fault. How he can legally chart notes in a midical/legal file, Is beyond me. But, she the abandoned him as a patient. No referral to a new dr, no response I they would support his seizure medications til then. We are in limbo with no neurologist to help with Nick's seizure disorder. Dr An abandoned him.
Desired outcome: Reprimand for patient abandonment. Apology from her and her staff. Honor the medical code of ethics and help him til he finds a new dr.

Fraud doctor Dr. Patel and fraud John C. Lincoln Hospital (Honor Health)
I spent two hellish days in that so called hospital. I was in the area two years ago and went to see dr. m. Patel for cancer. I wasn't there one minute and he groped me. I ran out of there and never went back. My cancer is now healed from juicing, I refused all conventional treatments, I had no surgery, no chemo, no radiation. I went to john C. Lincoln hospital, so-called, for a heart thing and dr. m. patel comes into my room. He stocked me and hunted me down. And was there looking at my breasts up and down. I screamed and told him to get out. I was not there for cancer but that would not matter, I did not ask him to come there and it is not good to up upset heart patient. WOW. Nurse Diane should not be a nurse, she yells alot. She yelled at a heart patient. They are incompetent and do not know what they are doing. RUN AS FAST AS YOU CAN. I DID. I RAN OUT OF THERE AND DIDN'T STOP RUNNING. THEY DO MORE TO UPSET THE PATIENTS THAN THEY DO TO HELP THEM. See what happened ambulance brought me here with heart problem. I was misdiagnosed but no cardiac surgeon showed up like they said he would. Instead an oncologist showed up saying to them that he is my doctor. He was never my doctor. I went to see him once for a few minutes two years ago and he touched me improperly so I ran and never went back. Here it I two years later im at hospital and he tells them he is my doctor and shows up in my room glaring down at my chest up and down and says, "Remember me" I screamed for him to get out. I was being held there unless I signed something to leave. Security guard stood in doorway so I couldn't leave. He then chased me stuck right to my butt all over the hospital and two other securty guards chase me too out into parking lot. My phone had died and couldnt call cab. They would not call me a cab. I knew no one in Phoenix. They chased me out into the streets. I was wondering the streets looking for someone with phone to call cab. Here I am a heart patient and they put me through the ringer. I had to go to another hospital 2 days later and they diagnosed me correctly with fluid build up in heart sack. I could have died. They had to take fluid out. I died twice they had to defibulate twice. That so called doctore told them that he is my doctor. That is fraud, He was never my doctor.
Restore vein wellness center
For several years, a business named "RESTORE WELLNESS CENTER" has aired ads on Phoenix TV that are very deceptive, false, misleading and just plain lies! These TV ads were produced several years ago, and several times during the ads, they show on one's TV screen in a large rectangular box for potential clients/patients to "HURRY AND CALL NOW! SCREENINGS ARE BOOKING UP FAST!" Already this morning since 5 A.M., I have seen these false, deceptive phony ads at least four times on channel 15 and again on channel 3. I have called the number they show on TV [protected]), but no one answers the phone at such an early hour! How then, can "SCREENINGS BE BOOKING UP FAST?" These phony, false, deceptive TV ads run about every 30 minutes in the very early morning hours, including Saturday and Sunday mornings when the joint isn't even open on weekends! During the past three or so years, I have called about 4 dozen times, and never received an answer. During any given weekday when they are open for business, I have called and asked about getting scheduled for a screening, or if they are already booked up as implied by the TV ads. I always get the same response that they never get completely booked up, that they always have plenty of openings on any day I would like to schedule an appointment! Now then, going back several years when these ads were produced, how would the producers know that several weeks, or months, or years later that "SCREENINGS WOULD BE BOOKING UP FAST?" They would not know! This medical organization is phony, deceptive, and fraudulent. Someone with some authority MUST investigate this deceptive, fraudulent business and force them to STOP airing these deceptive, fraudulent TV ads at once! A business like this will do their best to sell their clients/patients far more medical services and treatments than what they really need. I know this to be true because I got caught up in such a trap in 1978 living in Indianapolis when a chiropractor tried to get me to sign a contract to have monthly treatments on my back for FIVE years! Each treatment would cost me $500.00, so for one year, I would be paying them $6,000.00, and for the duration of FIVE years, I would be paying them $30,000.00! Needless to say, I did NOT follow through with this deceptive "plan!" Neurology appointment on February 2, 2023 at 2:15pm. When I was being called in, the nurse did not provide me an asl interpreter. I asked for VRI and they do not have it. They had one male nurse tried to sign but he doesn't know much at all. He did not translate me with neurologist but only can sign me what the neurologist said but he is not certified interpreter nor not know much asl. It was frustrated because there was no communication access for any languages there; no VRI.
Desired outcome: Negotiate settlement. Provide VRI for future patients.

Patient abandonment.
I was assigned through BCBS AZ to a physician Brian McConnell, MD in Green Valley AZ. I called to schedule a new patient visit but was told by the employee that he didn't do primary care. Several months later (August 2022); I made an appointment with the same physician for HRT. At the time I had forgotten Dr. McConnell's name regarding the PCP. During that visit I explained to Dr. McConnell that I needed a PCP and that I had been originally assigned to him. I explained to him that I have a number of chronic meds that I had been stable on for many years and that I only needed to come in 1-2x/year for BP checks and routine labs. He agreed to be my PCP though he stated he only did this for a chosen few. At around the 3rd week of November (after a few visits for labs etc) I called because I was running low on my refills and wanted to know if I should schedule an appointment for this or would he be willing to refill the meds without me coming in. The employee I spoke to said that the refills for this wouldn't be a problem. A few days later I got a call from what sounded like the same employee stating that he was no longer willing to be my PCP. He nor the employee gave me any explanation as to why I was being terminated and I never received written notice as required by law. Again that was late November 2022.
Desired outcome: I would like him to be disciplined in some way by BCBS AZ up to removing him from your preferred provider listing so he doesn't pull this again with some other unsuspecting patient.

I got an appointment to this doctor on January 10 2023 at 9 am. I came prepared and put everything in writing, I made the list of my vitamins as well as diet I follow and everything that could be helpful for a doctor. I met the nurse, who did not want to listen what I am saying, typing and making up her own conclusions. For instance, when I said I use to have my appendix removed when I was 10 years old, She typed and repeatedly said "10 year ago". When I corrected her, she did not even apologize to me, instead she asked me if I am OK? Then I said she should be more attentive what I am saying, her face went deeply red, she went angry. This wonderful human being left the room without even taking my vitals.
And I have heard, how she was complaining on me to someone in the hall...
Then doctor Lauren Geyer came over and I tried to explain to her the reason why I made the appointment today and when I asked, why her nurse was so rude to me. The doctor Geyer said that I am being rude. None of these "ladies"not even tried to apologized for being so unprofessional to me! Both of them are easily loosing their temper and might need some classes. None of them are listening or pay attention to what patient is saying! what is the point to go to doctors like this? Doctors who don't know how to effectively communicate with people? Who needs that type of doctors?

Failure to return unused deposit
I was required by the state to pursue child reunification counseling after divorce proceedings. The down payment was $5000 prior to starting services. My child refused to go and now the time limit has expired for the court order. I requested a return of the left-over deposit funds in October 2021 and have been getting the run around or zero response from the office of Dr D.J. Gaughn. The last communication received from the office was a promise to pay letter sent 9/20/22.
Desired outcome: Immediate return of funds.
Dr. Ajay Narwani
Dr.Ajay Narwan's office makes a patient get procedures in order to receive the pain medication they need. I was just told I had to get a procedure that didn't work last month (that I still owe $479.60 on) this month to get my pain medications. They pick procedures to do at random first it was shots in both knees now it's epidurals in my back. I told I have had them before and they did not work, he stated he needed to do them for the insurance company. There is much more about this office that is wrong.

AMR . MARICOPA, ARIZONA - I get a text twice every week, I have had a three-way phone call between my insurance, myself, and AMR here in AZ, I have sent letters to the AZ office and two to the corp office with no responses from any of them, I have made complaints to the BBB with no responses from AMR, the bill was not billed to the insurance company correctly, so I get paper bills and texts all the time, come on AMR this has been going on now for 2 years, WTF.
Desired outcome: Bill the insurance company correctly.
Doctor Scott Forrer in Yuma Arizona
Doctor Forrer was rude and aggressive towards my son on the last visit to his office. He was upset that my son did not get the MRI the doctor wanted him to get. But in fact it was his office that did not put in for the appointment so my son could not get the MRI done. My son called doctor Forrer's office several times trying to get the appointment but was always treated rudely and told, "they would look into it" each time he called but nothing was ever done. When my son went to his appointment on August 10th 2022, the doctor was aggressive and abusive towards my son. He made statements like, "I'm done babysitting you" and "I don't think you even have epilepsy, I think you are just having anxiety and need to man up". But my son has been diagnosed with epilepsy and has been being treated for it for several years. Doctor Forrer then kicked him out of his office and told my son that he was done with him and to not come back. This doctor's lack of professionalism and carelessness has now placed my son in a serious situation. If he cannot get his prescription to manage his epilepsy it could lead to serious medical problems. I looked into Doctor Forrer's past patient ratings and found numerous complaints about this doctor. I believe that he is reckless and borders on malpractice. His actions could lead to serious complications for his patients or even harm.
Desired outcome: I believe that Doctor Forrer should be investigated and at the very least censured, if not disbarred from practicing medicine. He is reckless and his actions could be harmful.

1) Not following the standards of care for vein retrieval. 1V: Groin to knee, 2 or more vessels: Groin to ankle
2) Not following standards of care for blood sugar monitoring. If Veteran is a diabetic, continues on current OP insulin AND ON REGULAR insulin AC meals, HS and 0300 starting coverage with Fasting Blood Sugar of 150
3) Significant Post op leg incision and sternal infections.
---Please evaluate the following care Mr. E 4223: AVR and 1V bypass 6/23, 6/25 developed acute vision loss, consult opthal on 6/27>who came to bedside and Dr. Madigan said not needed. Vision worsened, opthal returned: rec's for ESR/CRP, ACE, Lyme, b12,/folateMRI brain (given that Veteran also had documented PMR and on 10mg/Pred-worried over poss GCA/stroke), Dr. Madigan declined to obtain brain MRI/labs. Thereafter Dr. Madigan went on vacation and transferred Veteran to CLC (to be off his service), had anasarca/dyspnea, readmitted x2 (from CLC after 36 hour stay) under medicine as no CT surgery on service. Developed dehis, drainage of sternal wound. More than 12+ hospitals contacted in AZ, all refused to take pt for wash out, flown to CA. Had 4 sternal wires removed/2 washouts. etc. etc.
Desired outcome: Do not renew his surgical license. Unfortunately, this is not the first case and if you cared to look at other Veterans the outpatient wound care team knows and follows a lot of CT surgery wound patients.
