Letter sent to DrRalph Paylor, head of practice, Florida Eye associates, 502, E New Haven Ave, Melbourne, Fla 32901. This complaint concerns the business administrator of the practice, who instigated the patient discharge, it was not Dr Daniel Brown (Optometrist)who I have seen for years.2003 to June 2015 When you contact the office, the enquiries will go to her, (Trish Barker), who has totally ignored requests to call and explain.The person to contact in this this situation, is the person above her, Dr Ralph Paylor. Mailed copy sent to BCBS Formal Complaint Dear Dr Paylor, Since leaving Florida Eye Associates 2009 and since settlement 2010, with your practice .I have continued seeing Dr Brown numerously, for my eye exams. On June 4th 2015, I saw Dan (Brown)as I had a couple of issues and a diagnosis was given.I was highly recommended and given a referral and business card for Dr Parihar with Fla Eye assocs...Keep this in mind if Dan (Brown) had reason to refuse to care for me, why was I given a referral for another Dr within Fla Eye to deal with the issues and diagnosis I was given. 2 days later: On June 6th 2015, in the mail box, there was a notification card of a certified letter to collect, indicating it was off Florida Eye associates. Andy (Dr Zorbis, my partner )and I were leaving the next day for a month, so I had no time to retrieve it.Knowing also, the protocol for a certified letter is supposed to be resent, (if not picked up) and a replica letter is supposed to be sent regular mail, consisting of the same matter. Which neither took place. In October, on our return, I called billing to pay my bill, Jackie (in billing) informed me the certified letter was a patient discharge letter.I wanted a reason why (which by law, I have the right).She said, Trish Barker(business admin, , ) was sent an email to call me.After a week, of no response (as expected, I may add), I called BCBS. As a member of theirs with a PPO plan, my medical care was being denied, without explanation at this point.They (Larry, who also checked with a supervisor)agreed it was unacceptable. That day Larry, who I spoke with called Fla Eye and, he spoke to an Angie, who left him on hold, eventually forwarded to Barker and left a message for a return call, which she did not .Larry called back, to be told Barker was" too busy" to talk to Larry, Angie said Trish Barker would call me, Diane Prophett. I was told by BCBS if I did not receive a call, in a week, to call them back. Of course, no call! As expected. We went away again(with in the country), , still no call.Two weeks later, I called BCBS again as still no call with an explanation.This time spoke to Suzette at BCBS, she saw the data on this situation and she called Fla Eye, she spoke to Angie and was left on hold for over 11 mins, eventually was put through to Trish Barkers' voice mail and left a message, on my matter.Suzette called 2 days later and was told, she thinks, by Barker, that I would be getting a call.Told by BCBS, if no call within a week, to call BCBS again. I told Suzette, of your position, it was a Friday afternoon and remembered you would not be there.I told her I was going to put a letter together if this was not remedied before. Week later, still no response off Barker, notified Suzette, she tried calling again and left yet another message, stating if no response, they'll take it further.Suzette called me and informed me, she had left another message. Five days later, and a complaint has been made out to BCBS. The only person who has a right to discharge me with an appropriate reason is Dr Dan Brown, which I know, for a fact, he has not refused to treat me . Sincerely Diane Prophett (o3/09/1954) ss --- -- 0423 There actually is a longer version than this, as Dr Paylor is aware of the personal vendetta that Trish Barker holds against me, since 2004.There was a case I brought against them when I was terminated by her, that was settled 2010 (in my favour)but I am not allowed to speak details.Since then I have continued to see Dr Brown up to June 2015, who happens to also be a very dear friend of ours.He is not happy with the circumstances, but I told him, it is now in the hands of BCBS and she is discriminating me, without any rights or appropriate, reasonable reason. Thankyou for your help on this issue. I have a health plan to meet my medical care, I pay my premium and my insurance pays the Drs.This will be my up and coming 3 rd year with BCBS as very satisfied.This person has no right to interfere with my medical care and pulling my, file and discussing me, probably within the practice, there is most likely a violation of HIPPA too. © 2015 Microsoft Terms Privacy & cookies Developers English (United States)
