I made an online appointment at the VisionWorks location at Cumberland Mall (Atlanta GA 30339). That told me it would send an email to fill out online admission forms and did. Once I got the email I filled out the information the 5 tabs it showed.
Despite that when I got to the store for my appointment they handed me a form that requested the same information I'd done in the online form. They claimed they needed a "local copy". When I asked why it couldn't be printed from what I'd already submitted they laughed. I began grudgingly filling out the paper form at which point they handed me a second form to fill out. At that point I told them to cancel the appointment and left.
I attempted to complain via the 800 "Customer Service" number but was told they can't take complaints. They can only put me in touch with the store (apparently ignoring the fact I'd already been in the store).
Desired outcome: Not to have to fill out forms both online and in person! Why waste my time?

Neesha the dark skinned “manager” at store 482 in Snellville is a horrible sales representative. She lacks any type of communication with anyone. The only thing she does all day everyday is sit on her butt on her cell phone on social media and gossip about other employees. I was there all day at one point for my follow up and she didn’t look up from her phone once. And this happened four times in two weeks that I had to come into the store. Multiple employees and customers approached neesha asking for help and she redirected them to someone else. And this is what Visionworks hires as a manager? My 7 year old could do a better job than this. Because of this so called manager I know of, for a fact, five of my friends and family that went to Visionworks in Snellville and won’t return because of neesha. Do not go to this store! She is a horrible sales rep/“manager” and drove me directly out the door!
This is just to revering that all information is correct with the hoops form that needs to be signed every single time you see a doctor as well as the option to do the optos which is not covered by the insurance it takes a picture of the back of the eye for $19. Some patients feel like oh they tricked me if they don’t make it blunt that this is an option and you will have to pay $19 no matter what. Lastly the other form is if your okay with dialating your eyes with the drops, the doctor has to see the back of the eye to complete the exam.
