Quest Diagnostics - Tech Taylor A. assigned to 1361 Hylan Boulevard Staten Island, NY 10305

My name is Lourdes Acevedo. On January 4, 2021 I went to Quest Diagnostic located at 1361 Hylan Blvd, SI NY 10305. Tech Taylor A. was assigned to collect my blood. After writing down my required lab tests, she attempted to take my blood. She stuck the needle for the first time digging into my veins to no avail. She took the needle out and re-inserted the same needle a second time into my arm finally being able to withdraw my blood. To my surprise, my arm remained bruised for over a month. I had a second bloodwork appointment scheduled for January 14, 2021. When I arrived at the site, I respectfully requested that any technician besides Taylor A. withdraw my blood as I still had a bruised arm. Another tech withdrew my blood and everything was great. Today, March 1, 2021, I had another bloodwork appointment. Unfortunately, Taylor A. who by the way wouldn't tell me her name was going to withdraw my blood. Against my better judgment, I sat down and let her take in my information. After having to fill in the information for a second time, she proceeded to stick the needle in my arm, and just as the first time was unsuccessful at getting my vein to withdraw my blood. She took the needle out and I immediately saw that she was again going to reinsert the same needle for a second time, I immediately stopped her from doing it. I requested another tech withdraw my blood. She got angry with me and stormed out of the room. She literally threatened me stating to come outside as if she was going to fight me. Then she said she was going to call her supervisor on me as if I should be scared. Tech Lani withdrew my blood. I now have bandages on both arms because of Ms. Taylor A's incompetence. As I was leaving I asked Taylor A. her name, as she doesn't wear a name tag, and she refused to give it to me stating she doesn't have to give me her name. She told me that she had nothing else to say to me. I am a 62-year-old woman who doesn't need to be threatened by your employee. Her behavior is unprofessional at best and needs reprimand. She also could use retraining at withdrawing blood. I have never been treated in this manner and am very upset at this incident. All of the techs that have drawn my blood have been respectful and professional. Please forward this complaint to management as I am shocked. I can be reached at [protected].
