Panera Bread - incorrect w2

I worked part of the year at the Oak Ridge, TN Panera Bread.
I received my W2, and it shows an incorrect social security number for me, which I attempted to correct multiple times with the manager while I employed there.
Additionally, though it clearly shows I claimed 2 exemption, it does not show one cent held for federal income tax.
Please send me a corrected form, ASAP.
