On 10/2/23 I went in to order a Mod Salad. I asked for spicy chicken and got raw sausage instead. In addition, I asked for mushrooms and got some that smelled brown. I asked for them to be removed and give me the fresh ones on the other side that were still white. I told the manager and staff that they need to be very careful with mushrooms. They do not want people sick from food.
Desired outcome: The mushrooms were removed and replaced. Unfortunately, when I got home I realized the meat was incorrect.
