Arrived at 0830 Sunday morning. Husband having psychotic episodes, wandering, etc. c/o chills. Prior history was hospitalization for chills/fever, sepsis. Labs drawn at 0850. Sat and waited until noon when I was told, after approaching a nurse, "you just have to wait if you want help." Nobody else spoke to us. My husband is 77 years old with dementia. He...

I went to the ER on Saturday in the evening. I was having severe abdominal pain and nausea. I get these attacks that last for 15 hours, cause me to moan, hallucinate, I can't sit still, I sweat profusely and am extremely cold. My BP was also very high. That is not normal for me. Instead of your check-in nurse caring about my symptoms, she made the...

Mercy San Juan Medical Center The death of my husband

My husband was taken to Mercy San Juan hospital in November 5, 2022. He was clammy, looking like he was going to have a heart attack. The next day he calls me sounding fine. And that they are going to discharge him. A nurse called me asking what If he has a heart attack. I told em then you do everything you can to keep this man alive. She replied with well we don't recommend that. I said what? She tried talking me into just giving my husband a DNR. I told her no and that's final. She said ok hold on. And puts the Dr. On the phone. He tells me he's getting tired of treating my husband. Next thing he has a heart attack. I find out later they gave him medicine that he's allergic to. Epinephrine. It even states he's allergic to it. But the day they gave it to him it says allergies unknown. But on every page it clearly states that. Then after getting out of ICU he'd scheduled to go into a skilled nursing home. But then his blood pressure is dropping along with his heart rate. I find out they gave him medicine that is not even FDA approved yet. And then the nurse came in to say the Dr ordered a constant drip of morphine. I said wait. I want to talk to the Dr. She said he's busy but I can answer any questions you have. I said I'm sure you can. But I want to speak to the Dr. Never got to. Next day he was dead at 7am. They didn't call me til 3pm to tell me this. I get there and he's still in the same bed not covered up and with his mouth wide open. After he passed. I have the hospital calling me asking me where my husband is at. Then the insurance company that works with the skilled nursing home was curious where he was the hospital couldn't give them a straight answer because they didn't know.

Desired outcome: I'd like to hear what you guys have to say about this. Why was my husband given epinephrine? Why medicine thats not even FDA approved? It's not up to them to kill my husband. It is up to God.

Mercy San Juan Medical Center Breast reconstruction surgery (post bi-lateral mastectomy) negligence

I went through a bi-lateral mastectomy and direct to implant reconstruction on 11/9/22 @ Mercy San Juan. Dr. Jakobsen performed the mastectomy, Dr. Zhang performed the reconstruction. Dr. Zhang's team failed to prescribe me antibiotics post surgery; additionally, he failed to close an open channel across my chest between breasts during surgery. As a result, I developed a staph infection in my left breast (which antibiotics would have killed), was admitted to the hospital for emergency surgery 11/22/22 and had a washout procedure and removal of my left implant on 11/23/22; Dr. Black was the surgeon on call who performed my surgery and discovered the open channel during the washout procedure. She was then forced to perform a washout on my right breast as well due to potential traveling of infection through the open channel; she then also closed the open channel surgically in my chest. Dr. Black was able to salvage the right implant and tissue, however I was left with an expander in my left breast as a result of the infection.

As result of the carelessness of Dr. Zhang and his Nurse Christie Sang, I have incurred additional medical debt as well as pain and suffering and I still have to have another surgery to remove the expander and put an implant back into my left breast. This has caused me 2 additional surgeries that were completely unnecessary and avoidable.

Desired outcome: Internal policies and procedures need review. I would like to have my medical expenses from 11/23/22 and my forth coming surgery to be paid for in full. I should also be compensated for pain and suffering.

Mercy San Juan Medical Center Emergency room/ surgical floor staff

On December 12, 2021 I went to Mercy San Juan Medical Center emergency room because I was suffering with severe dental emergency a filling had fallen out a month prior but was not giving me any pain so my dentist and I decided to wait until the new year rolled around to do work on it so I would have my full benefits however Saturday, December 11 I woke up with swelling on my face and severe pain in that tooth I had a telehealth appointment with my primary care because the dentist office was closed they prescribed me antibiotics on the 11th in the morning which I took as prescribed all that day and night the morning of December 12 I woke up and my face was the size of a softball I couldn't open my mouth and the swelling had moved not just on my face but but my jaw my throat and right under My eye on the left side. So I called my primary care as well as my dentist him both advised that it is now a medical emergency and I needed to go to the emergency room as quickly as I could so at 11 AM I arrived at Mercy San Juan medical Center severe swelling and severe pain with a high fever difficulty swallowing and limited breathing because the swelling had moved to my throat and neck the triage nurse had no regard for anything that I was explaining to her and basically put me in the rapid care line so I waited to see the doctor through rapid care then after seeing that doctor he ordered intravenous IV anabiotic's and a CT scan with die of my face and neck area so that he could see how deep the infection was as well as bloodwork after several hours of sitting in excruciating pain in the waiting room because I was sent back out after seeing the doctor in rapid care was finally called into the back where the doctor proceed to come into my room and try to inject me with nothing in that area I could barely open my mouth as it is but he attempted to get me none anyone who's had any dental anesthetic put in their mouth knows that it's extremely painful when they just shove it in extremely fast and they don't do the shot slowly my face was already swollen when they injected the numbing anesthesia it left a huge bubble of fluid on the inside of my cheek and gum which made it that much more uncomfortable I was in tears crying in pain the doctor finally agreed after six hours of being in the emergency room that I could have some pain management they gave me 1 mg of Dilaudid and some Zofran from my upsets stomach as it made me nauseous. Shortly after that the ER doctor came in and explained to me that they had spoken with an ENT specialist and that the specialist advised I be admitted to the hospital because the infection had already abscess so deep in my cheek and jaw bone and spread to my bloodstream and my white blood cell count was three times the normal amount they immediately started me on IV fluids and IV antibiotics as well as a shot of anabiotic‘s while I was in the emergency room I witnessed two nurses who were blatantly disrespecting and speaking bad on two patients that were in there for psych issues one of the nurses stated about one of the psych patients when another nurse to asked where she was she was probably in the bathroom killing herself I thought that was completely inappropriate for a nurse to say about someone who is having psychiatric issues there was a second psychiatric patient it was in the room next-door to me who came in overdosed and they had to give her CPR and pump her stomach as she was coming to she was a little out of it and acting out slightly one of the nurses came out and was talking to another nurse and said doesn't a stupid idiot no we just saved her life! how are you going to be a nurse and talk about patients like that I understand they see a lot of homeless and low income families but no one deserves to be talked about or treated like that. Then as I'm going upstairs to the surgical floor I let the nurse know that my pain meds were wearing off and the pain was coming back to an excruciating level seems has it had been four hours since they had given me the pain meds I didn't think that they would have a problem giving me another dose even if it was Motrin or something anything to keep the severe pain from coming back I was desperate the nurse in the ER said that she would make sure that the doctor put in the orders because I was on my way transferring upstairs to my new bed when I got upstairs the nurse Chester who is a very nice guy came in and said that I'm sorry your doctor will only prescribe you one 500 mg Tylenol for pain I explained to him that one Tylenol was not going to work that Tylenol was not going to even touch the kind of pain that I was in with this abscess and the swelling and the tenderness and that I needed something more I explain to them that what they gave me in the emergency room worked really well and I would appreciate it if while I'm in the hospital they could continue that regiment until they did the surgery on my face the following day when they were going to Lancet under my chin and relieve the pressure after 30 minutes of going back-and-forth with the nurses and the nurse is going back-and-forth with the doctor the doctor eventually agreed to give me one 200 200 mg Motrin I couldn't believe it I was completely and totally back to full-fledged pain I was in tears and I was begging for them to keep me comfortable and they were refusing to do that I still had almost a full anabiotic prescription at home that my doctor had prescribed me the day before after finishing two bags of IV antibiotics and getting my shot I left AMA from Mercy San Juan because they refused to respect me and keep me comfortable in a time of great pain they treated me like I was a piece of garbage someone who didn't deserve to be comfortable in a painful situation they made me feel horrible like I was some kind of addict for asking for pain relief 😮‍💨 I hadn't been to the hospital since 2009 before that so it's not as if in their chart they had seen history of me coming in and out of the hospital I'm a fairly healthy person I haven't been in a long time all I was asking was to be made comfortable through this very painful experience I wasn't asking for anything more I just didn't wanna be in pain while I was sitting in a hospital in a place where I'm supposed to be treated safe and not be in pain also as a footnote when I left I told the nurse that I would like to speak with the doctor because he kept telling me that she was extremely concerned about me leaving against medical advice yet she refused to come and speak with me on the phone or in person she refused to keep me comfortable and even when I met her briefly in the emergency room before I was transferred upstairs she made no effort to introduce herself or even let me know who she was she popped in the room got on the computer smiled and walked out I didn't even know that was my new doctor until I got upstairs and was told that the lady that I saw in the emergency room when I came up here was her. I am so disappointed with how Mercy San Juan treated me not as a patient but as a person in general they made me feel like I was nothing and nobody when they are sick and feeling as horrible and painful as I was should ever have to have someone look at them and tell them that they're not experiencing the pain that they're feeling and that one Tylenol should help I don't even take one Tylenol or one Motrin when I'm on my period every month I take at least three Motrin's so why were in 1 million years with doctors and nurses believe that one Motrin or one Tylenol would be enough to take the pain away from something as excruciating as the type of infection that I had going on it wasn't even considered a dental emergency anymore it was actually considered a medical emergency according to the ENT specialist because of where the abscess was located and how severe it had transferred to my bloodstream and still with the CT scan with the facial swelling with the tears I was in with all my complaints and all the proof they had that it actually really was happening they still refused to make me comfortable and that is completely unacceptable when it comes to hospital and patient care especially when your slogan is human kindness

Desired outcome: Proper training for er nurses with regards to homeless and psych patients and A permanent write up in the file of DR. Suang.

Mercy San Juan Medical Center ICU RN Mike

Hello,
My sister, Raina Thomas DOB 10/24/73, was a patient in the ER and ICU on Saturday August 28 and Sunday, August 29, 2021. She felt unsafe and received hostile communication from RN nurse Mike. I called to address this and spoke to Mike's supervisor, RN Aurora. I was never given last names so I cannot include those. I explained to Aurora on Saturday evening that the hostile and at times aggressive and passive aggressive treatment my sister described and I needed it addressed as soon as possible as it is unacceptable. I also requested a change in nurses for my sister which Aurora said was not feasible. Aurora was professional throughout my conversation with her. She attempted to justify Mike's care as excellent and someone she would want taking care of her family. She stated she would address Mike's behaviors and speak with my sister that evening. I explained that if Mike's inappropriate patient care continued I would move my grievance forward and notify the board as patient's must feel safe. Aurora never followed up with my sister that evening. She may have been dealing with emergencies and I understand that. My sister kept me on the phone and I witnessed an interaction with RN Mike after asking that the hostility be addressed and it continued. At that point I directed my sister to request another nurse. The nurse who then helped my sister, although my sister does not remember her name, was wonderful per my sister's report. I think it is vital that a patient be treated with care and respect and feel safe. The treatment provided by RN Mike needs to be investigated because as soon explained to Aurora it would be uncommon for this to be an isolated event.

Desired outcome: Training on patient care and safe environment for patient

My cousin came in having a heat stroke and they instead diagnosed her with pneumonia and a bladder infection . Wow I just wonder where they got their license to medicine. She had every symptom of severe heat stroke and they came up with that .they must get some funding for pneumonia patients because 3 other people I know came in and they said the same thing for them . not one was having problems breathing .

Mercy San Juan Medical Center The service mother received and the grief I endured.

Your ICU Dr. Murphy highly suggested that I allow him to give my mother YOSHIKO PARKS a morphine drip so she could die, as he related he had done so with his family and it was for the best, also Dr. Bestrong was ready to bury my mother, he made it a point to show me her lung chart and pretty much said zero hope. It's funny that all of your Caucasian Dr. are so very text book, all of the non Caucasian Doctors still had hope. Your 2 Doctors severely affected me mentally and I insist on rectification. My mother is doing very well, at the end of January 2021 she was at a level 95% health, my mother would not be here today should I have listened to you so called professional Doctors. ...my name is Michelle Stanfield I am mothers POA, she was in your facility the 2nd week of September 2020 to mid to late Oct.2020, I was given more Info from the specialists who came in to work with her, such as that she shouldn't be laying backwards in her bed due to her double phenoma, your Doctors who saw me there for 1 month never told me this as I was there everyday from 10:00 am to 10:30 pm every day for a month, I think you need to seriously question your so called specialists. I'm grateful to be able to share time with my mother, had I left it up to your specialists she would not be here today.

Mercy San Juan Medical Center landscaping crew

Yesterday I arrived at the hospital at around 6:20 in the morning. I parked my car very carefully as it is a brand new car. As I walked in I could hear a leaf blower going from the grounds crew. I didn't think they would be blowing around cars but as I looked back he was indeed blowing around the cars one aisle over from where I parked.I came out later to find my car covered in dirt leaves and a nice big surface scratch. I want this to be forwarded to the appropriate person. I would like for your company to pay for my car to be detailed on the outside and hopefully have the surface scratches buffed out.

Mercy San Juan Medical Center emergency room

I was taken by ambulance to Mercy San Juan emergency center on April 1st at approximately 5 PM. I had fallen face first on my stone entryway and barely crawled to my phone. I was barely conscious and managed to call my neighbor. She came over and I was hardly conscious and going into shock with blood all over me and the floor. She called 911 because I wasn't responding to her and couldn't speak. The paramedics took me to the ER. Once there the paramedics took me in and told the intake nurse I fell and she did not even look at me. The paramedics stuck me in a wheelchair and put me in the general waiting area and left. To summarize, I came in on an ambulance with a head injury and the intake nurse didn't glance at me and the paramedics put me in the general waiting area. It wasn't until I started projectile vomiting that someone came to look. The nurse that came out made the comment "she came in by ambulance with a head injury. This is not right". She was referring to the fact that they simply stuck me in the general waiting area. When I left the ER I had broken the bones around my eye.
That is unacceptable behavior by the intake nurse and paramedics.

Mercy San Juan Medical Center emergency room. urgent. pain.

I am in horrific pain. I was in a car accident last month on 6/14. Went to ER on 6/17. CT scan showed left breast silicone implant ruptured and bleeding above left kidney near Petit Hernia mesh repaired area from previous surgery. Told the Trauma surgeon and attending physician about pain in pelvis. I was sent home. With Norco for pain. I have been complaining of severe pelvic pain to my doctor since that ER visit.

Flash forward a month: nothing done for pelvic pain. It was so bad I couldn't sit down or stand at work on last Thursday.
So I went to ER. Explained pain. The man ordering images did not listen to me. He ordered the same ct scan that I had on 6/17. I tried to explain that the pain is pelvic. It's severe. It's Not my back. Not same injury. I have 2 injuries seen on ct scan 6:17 due to accident. I have had pelvic pain since accident. It was NOT the right imaging I needed. The man that ordered it was not listening. His reply was " this will show everything". I am 58 years old. I know better. He was so rude. So arrogant.
Had blood work done. Urine too. Waited another hour. Brought to room. Had a IV stuck in my arm. Still sobbing. Shaking too.
Waited another 45 minutes. Left on a gurney crying in pain. People walking by. Nobody noticed.
Got in for CT scan and they suddenly noticed an iodine allergy. So. The CT contrast scan couldn't be done. After they have all my info from ER visit on 6/17. Pathetic. The allergy is listed on my chart. Got a regular CT scan. I already had that done on 6/17. I have appointments with surgeons set up for those. I was there for Internal PELVIC PAIN. I told the tech and she said the CT scan might show it but I need other imaging. I KNEW THAT. This CT scan wasn't anything new. Keep in mind this whole time I am crying from pain.
I'm then wheeled back to the room. I waited for an hour for doctor. She came in. I'm sobbing. I'm shaking from pain. She said "what are you expecting from me today? Wow you have a big blood clot on your back." I knew that. I said " I HAVE SEVERE PELVIC PAIN FROM ACCIDENT". Her response? " Well I think I should go look at your images myself". Oh my God. I just couldn't believe it. Then she said I might need a pelvic exam. Told me to take my underwear off for pelvic exam. So I waited. In pain. Sobbing. After 45 minutes and no sign of anyone I was in so much pain I got up and walked out. Sobbing. Still had the needless IV in my arm. Had a nice lady remove it at the station. I told her why I was leaving. She wasn't surprised. I have never ever been treated so poorly in my life. Nobody cared at all. Nobody did anything to make me comfortable. I had no pain meds with me. Nobody went over my lab work with me. Don't say it's because I walked out. They had more than enough time to talk to me. Nobody reviewed my imaging with me.
I pay a lot of money for my insurance. I work very hard. I have nobody to take care of me.
I need surgery. I am injured. That accident wiped me out and I'm having to wait for surgery consults. This pelvic pain is horrendous and the only pain meds I have were from a dental surgery.
The pain I was and am still in deserved X-rays or an MRI. Not the same CT scan that I had last month.
You have all my records. On site. In computer. You have my imaging and info from ER on 6:17. NOBODY LOOKED. NOBODY CARED.
I called my doctor to let him know I was going to ER and why.
I called him on Friday to tell him I am in horrible pain. The ER doctor should NEVER allowed me to suffer. It was obvious that the pain was horrible. Shaking. Sweating. Crying.
I have been home in bed for 4 days in so much pain. I'm scared. I would go to the ER but they didn't do anything. I can't go to Med 7 because they send me to the ER.
I NEED SURGERY. I AM HURT. I WAS REAR ENDED IN CAR ACCIDENT. WHY DIDNT THE DOCTOR ADMIT ME ? There is something very wrong. How is it so far fetched to understand that the accident has injured the left side of my body and the pain I'm in deserves an X-ray and MRI? I begged someone to listen to me. Nobody did. I need help.
And I was presented with the bill: $1080. I am NOT PAYING THAT BILL. I AM NOTIFYING BLUE SHIELD. And my attorney who is representing me for car accident.
If the car accident has caused 2 injuries caught on CT scan, then WHY would a doctor not do something for a 3rd injury on same side of my body? That is so negligent.
I am still in pain wondering how I am going to be able to work tomorrow or any day with pain at the level I'm experiencing.
You need to do something. Quickly.

Mercy San Juan Medical Center hospital

Not the first time.
My daughter has MS and has been admitted alot of the past 4 years.
Most of the staff doesnt even know what MS is nor are they familiar with MS medications or the risk of infection that goes with it.
When she is admitted they give her pain meds if shes in pain which is needed, but I have to pull teeth to get test done. To find the issue. Fix it and get her off pain meds.
They seem to give people pain meds without finding cause send them home accuse them of drug seeking .
The system is flawed the doctors hands are tied authorizing test.
There are good nurses and some that are horrible however most hospitalist Doctors are not doing their job.

Mercy San Juan Medical Center treatment in the er

To whom it may concern,
I am writing this because I have been treated very poorly by this ER department. I was seen on 3-2-2016 for severe stomach pain and vomiting of blood. The doctor who treated me was kind to me, but blunt. He said I needed an endoscopy within a week. He said my stomach may have a malignancy after reviewing my CT scan. He was willing to admit me to the hospital immediately if I thought I could not get to a doctor. I felt that there should be no problem getting in to a doctor, and I didn't want to take a bed away from someone who might be sicker. There were so many in the ER department that day. I left, and consequently made no less than three full-day trips to the ONE provider that would accept my insurance, never being seen. The last trip, I was told that I hadn't signed in on the sign in sheet at 5 pm. I had been there since 9, and my name was at the top of the list. Needless to say, this upset and discouraged me GREATLY, because I felt that I was being wrongfully dealt with. After much research, I concluded that no doctor in this area would accept my Molina, and headed back to the ER after a night of vomiting black gross stuff. I was completely honest with the staff, declined pain medication, and patiently waited for hours to talk to the doctor. When I was finally called back, the doctor was a monster. I don't even know what his name was, because his badge was turned backwards or something, all there was on it were the letters DR, but I'm sure they have my record of this visit which occurred on March 23, 2016. He never looked me in the eye, or let me show him my discharge paperwork from the previous visit stating there was a possible malignancy. He was brusque, short, asked me about my medications and abruptly walked out the door when I stated I was taking methadone. I was brutally attacked and raped in the year 1994, and from that attack I had a child. There were a lot of problems during and after the pregnancy, which was NOT overseen by any doctor or healthcare, and I have had severe problems ever since, including an eventual complete hysterectomy. I was on painkillers for over a decade, and the dose kept going up to deal with the increase in tolerance. I made my OWN decision to stop taking all the different pills, and put myself on methadone with the help of a clinic. I should not regret this, but now I find myself being treated like a piece of garbage by a doctor who should have respect for someone who chooses to suffer instead of be an invalid on a plethora of prescriptions. I was tired, and sick, and at the very end of my rope by this time in the ER, and I lost my temper and spit at the floor by the window that he was sitting behind. I did not deny it when asked. I was immediately escorted off the property with no paperwork, and I accept that. I lost my temper in a totally unsuitable way. But MY GOD, WHERE IS THE COMPASSION HERE? HOW CAN YOU TURN AROUND AND WALK OUT ON SOMEONE WHO IS TRYING TO TALK TO YOU RESPECTFULLY? I didn't lose it until he walked away from me. I don't even know this man's name, but I imagine it's on the electronic records of that day. I am the first to admit I was wrong. I shouldn't have lost my head so badly. But I was having some pretty terrible thoughts all of a sudden, and I've never been someone who felt like taking their own life. I wanted to hurt him for hurting me so badly. I spit on a window. I'm still crying over this stupidity after HOURS. Please, talk to the security guards who walked me off. Ask the triage doctor if I was a monster. Or was I a 37 year old single mother of two who is afraid she is dying slowly? I don't know a lot, but I do know that every person has their limit, and after the double humiliation of being called a liar after over 20 hours spent waiting for a regular doctor at the wellness center and then meeting Doctor Charming and being blown off like a piece of dirty tissue, I seem to have reached mine. I have never been in trouble in my life, no arrests, no issues. I don't deserve to be treated so badly by someone who is sworn to help me? NOT if I'm being honest, and patient, and NOT if I'm afraid IM [protected]@$%NG DYING! MALIGNANCY IS A VERY BAD WORD. HOW COULD HE BLOW ME OFF AND NOT CARE AT ALL? If you are a doctor and you have that much disgust for people, you should go into podiatry and stay away from the ER. I lost my cool. Please stop this doctor before he makes somebody else do worse than spit on a floor. I'm personally shocked and disgusted by my behavior. I know myself, and doing something like that is NOT me. That is why I've written this letter to you. If I can be pushed to spit on a floor, God only knows what other grief this doctor has caused with his horrible attitude. If I am not the first to complain about him, then maybe this will be another drop in his bucket. This man is going to drive someone to hurt themselves or go after him. It's wrong to have him in the emergency room. I apologize to the staff of the ER for losing my temper and my sanity, and I am ashamed of my juvenile action. But it was harmless compared to what he did to me and my well being, which was in a bad way anyway. I've seldom felt so hopeless as I do right now. I only ask you this: If you had been unable to eat, or rest, because of a deteriorating stomach condition, and you had been told that you should be admitted to the hospital immediately with the word MALIGNANCY on your paperwork, and declined the admittal to try to go about the thing the proper way, if you then spent the next couple weeks sitting all day long THREE SEPARATE TIMES in a place that denies you are even THERE the last time, spent FOUR phone cards calling offices that didn't ever call back, then in desperation went back to the place that had made the diagnosis, and were dismissed and blown off in a nasty way after hours of patient sitting and yet another blood test...how would you feel? You might not be as juvenile as I was. Or you might have been worse. Who knows until that horrible time comes for you? I apologize for the long letter, and the only thing I want out of this is to make a complaint against the doctor who saw me. The ER staff just naturally overreacts to things I believe. I think it was excessive to act the way they did but whatever. Maybe they didn't notice that I spit on the floor with the guy on the other side of the WALL. I don't know. But I do know that that doctor has no business in emergency medicine with the smug attitude he affects with his patients. Be goofy. Be remote. Be businesslike. But for God's sake, be HUMAN. This man is not.
Sincerely,
Dawn Ray
