At approx. 6:25pm Driver driving vehicle number 33628 out savannah GA area, was in a left turn lane to turn on to the I95 northbound in Richmond hill GA, intersection HWY 17 iI95 Northbound and driver decided to cut off a motorcyclist with only 30 feet or less of room causing the motorcyclist to have to lock up the bike to avoid collision. Very unsafe driving.
drug screening
My pain management doctor told me in order to keep her license to dispense pain medication I must have routine drug testing by federal law, my insurance did not pay for this and Quest charged over 1, 000 dollars each time. bloodwork being handled incorrectly
On Thursday, February 28, I was sent to Quest Diagnostics location, 201 S. Buena Vista, Suite 125, Burbank, CA 91505. Upon arriving, I followed the instructions and soon the person came from the back to get me and two others. I felt like herded cattle. The woman (who didn't give me her name) yelled something toward my way, which I assumed meant to wait in the other waiting room down the hall. The waiting area had a taped sign on the door that stated to keep the door open at all times. The walls also were riddled with holes from previous items being hung there. A long time had passed, so I got up to stand in the doorway. She came out of room two which I noticed was the main workspace. There was another dark haired woman working across from the woman taking care of me. The woman taking care of me was heavy set and had hair dyed with Blonde and Carmel.
Once the woman found me in the system and keyed in my order, she was ready to begin. It was getting later in the afternoon, as I came straight from my doctors office. It was after 3:30 p.m. The woman of concern, once again didn't tell me her name. I asked several times and she said it so fast, I couldn't understand her. I had to tell her several times to slow down because I was having trouble hearing her. This was the first time I seemed to upset her. The second time came was when she was about to put on her gloves to begin. I asked her if she were going to wash her hands? She replied with, "I will if you want me to!" She turn returned to me and proceeded to look at a vein on my right arm that was the smallest one on my arm. BTW, I have great veins. She pulled out a huge needle. After moments of her not doing anything, I looked up at her and she chose a smaller needle. I guess we could call this number 3. She then proceeded to stick the needle in my arm, pull it back a little, then dig deeper until she got blood. The attached photos will show you the days that follow how horrible my arm looks and hurts. I think she does not need to work in this industry.
Neither she nor her coworker washed their hands at any point in time. They were constantly either touching their hair, noses or skin in general.
Another thing that surprised me was the fact that what appeared to be bloodwork of the day for many patients was each in their own bags, however, each bag was open and on the desk of the brown headed coworker leaving opportunity for contamination of all sorts to enter each bag.
Needless to say, I won't be back at this Quest location. I usually go to one closer to my doctor. The only reason I went here was because this location offers X-ray as well.

I was driving north bound on I95 in Bridgeport CT behind driver 75674 during normal rush hour stop and go traffic. The car was swerving back and forth in the middle lane tailgating the car and box truck in front of it...the cars that were in both lanes next to it did not want to drive ahead of the quest car because they were driving so erratically. I'm not one to complain on sites but it was so bad and unprofessional I was appalled!

The phlebologist was not ready to do the procedure seemed totally unprepared kept opening and closing the cabinet drawing looking for bottles where the hell is the rack with bottles. Touching surfaces it not allowed for in a procedure. I guess you don't ever wash the hands even if you were asked, right? How do I know someone did not poop and then draw my blood there's no one there to oversee the operation. 2 When you go through a vein completely you carefully back and if you don't see blood you pull it and start some where else even a junkie know that much. You don't ream it sideways, up and down and in and out, because, you pierced it and it has to recover. You start some where else. Why don't you send your employee to a shooting gallery and watch the junkies so you can learn how to get it right. Now I don't what desease you have spread to me and I need a lawyer, why not. I think everyone should get a letter that received blood on 01/11/2019 at Oaks the regular girl is off and you put someone who is probably very scared and nervous and that's really shame but what about us. So think about about it tonight when go for steak at longhorn the cook goes and takes a nice bowl movement and put a swish, swish of hand sanitizer the picks up that steak and moves it around plate with his fingers. Oh wait, he required to wash his hands but Quest is not.
overbilling for people without insurance
I had labs drawn in September of 2018. To my dismay, the insurance I had refused to cover some of them. I didn't realize at the time that the prices Quest gave me if I had to pay out-of-pocket were ridiculously over priced and unfair. They will not negotiate the pricing either.
IF YOU ARE PAYING OUT OF POCKET FOR LABS, OR YOU THINK YOUR INSURANCE COMPANY MAY NOT COVER SOME OF THE LABS,
DO NOT GO TO QUEST DIAGNOSTICS UNLESS YOU WANT TO PAY UP TO 10 TIMES WHAT OTHER PLACES MAY CHARGE FOR OUT-OF POCKET COST.
SHOP AROUND AND LOOK FOR REASONABLE CHARGES OR YOU WILL BE UNPLEASANTLY SURPRISED.
fraudulent billing
Quest continues its fraudulent billing practices. Recently I received a collection notice from Quest for over $1, 300. I had made a co-payment at the counter when my physician referred me in for blood work. BlueShield Ca had already settled the claim. Now, I am being billed for another $130.00. I already made the co-payment.
I have repeatedly asked my physician for an alternative to Quest. In reading the feedback regarding the company, it appears they had a class-action lawsuit on this topic and yet they continue the practice.
driver
This evening I was driving home and the driver of vexhicle 75431 ( didn't get plate) was driving very recklessly. This occurred in reading Pennsylvania merging onto rt 222 and then again on rt 222. The driver accelerated toward me as I was merging into traffic I swerved back into side of highway. He then proceeded to almost rear end the car in front of me. We then merged into the bypass where he again was riding the car in front of him way to close having to apply brakes to slow down. He swerved into other lane and back into the lane ahead of the next vehicle he continued to ride the bumper of the next car until I got off at Lancaster ave ramp. He was still driving unsafely when I lost sight of him. At the rate of speed and recklessly driving he could hurt someone very easily especially in the dark and with holiday traffic. Unsafe driver needs to be reminded that his actions could cause someone to loss there life.
unethical behavior
Cynthia, a home lab collector, was extremely disrespectful and rude. After having a heated confrontation with her, she purposely sabotaged my urine by throwing it away. I filled a urine cup over the tempature line, up to 30ml in fact. I requested she submit to the lab because I knew she was giving me a hard time because of our arguement (which is horrible customer service). I then called for another staff member to look at the amount and she poured the test out. If anything negative comes of this, I will be contacting an attorney. Does Quest even do background checks because this woman is nasty and rude.

Went to my appointment and waited over an hour and finally was told they did not have any orders from my doctor. I went home and called my doctor who immediately faxed over the order so I would be good the next day. I went back and the next day and told them the doctor had faxed the order over and I need to add an additional test from my onocologist Dr Mortazavi. He want a special test that he didnt think my primary would have on his order. He wanted to piggy back on Dr Zwolak's orders. When I checked in I told them that this request was to be added to Dr Zwolak's order. Again I waited over an hour and when the technician came he pull one small tube of blood and said ok you good to go. I said no I am not you only did the test I brought in to be added to Dr Zwolak's orders you need to find the orders. He went looking for them and said he found them miss filled under the day before. To make a long story short he did the original order and sent me on my way. They had all my billing info they made copies of both my medicare and my blue cross addn'l insurance. Now months later they are billing me for two separate visits when I should only be billed for one. I should be billing them for wasting my time.
Please correct this error.
billing blood work improperly
Blood drawn in January 2018- confirmed Quest labs in Bethlehem, Pa was in my network for Highmark BCBS. Confirmed again in the office when I provided my insurance card to the tech. Since then, I have been receiving bills stating that my insurance denied and I am responsible. I have called over ten times to provide the SAME insurance information of my policy to many different customer service reps. I have spoken to Highmark as well who reassured me that they are billing incorrectly. Highmark rep called Quest with me on the phone, explained to them exactly how to do their job, and now I'm being told I'm being sent to collections?! This is fraud.
reports not sent to doctor or myself - negligence
November 8, 2018 Complaint regarding negligence in forwarding lab reports
I did blood collection at Quest on 26th Street in late October. I was told this was enough time for the analysis report to be completed and sent to my doctor and filled out a form for a hard copy to be sent to me which I gave to my practitioner Mariana (not quite sure of her name).
My appointment with my doctor is tomorrow. I wanted my lab copy in hand for consultation with the doctor. No copy arrived. I called Dr. Levine-Tanco's office, Mount Sinai/Beth Israel senior health services, 8th avenue at 24th Street, they did not get their copy either.
I called Quest Customer services and was told they would fax a copy to my doctor immediately.. My follow up some time later revealed no copy had been received.
If this were the first time I might consider it an oversight, but this is the second time this has happened and I am considering changing lab providers. The technicians have been wonderful but follow through is non existent. I have complained to Quest and to Medicare. Improvement is mandatory.

The quest diagnostics website is absolutely the worst of the worst, and quest's customer service is non-existent. It is obvious that quest does not care about its customers/patients. Forget trying to contact quest by telephone — that is, unless you want to pay a bill. Quest does not provide any customer service phone numbers. I had routine tests done on 10/18/2018, and I have been unable to get the results. The foregoing review solely provides my personal, subjective opinions, beliefs, and interpretations.
could not get my results but got the bill
Lab work done in March, my doctor could not get the results because the lab work could not be found and I could not get the results but I got the bill over and over.
I finally paid the bill and spent an hour on the phone to get the results to take to my new doctor.
I was told that I would have to get my old doctor to call and correct the demographics and I probably would still need to have the blood work repeated ( second time free).

pre-employment
This company needs to be investigated for discrimination. I applied for a position with this company a few months ago, and was offered to be interviewed TWICE, only to find out that I would not be considered for the job. I'm not upset that I wasn't considered for the job, however, I am upset about the truth behind not getting the job.
I found out today, on 10/17/2018, (when the RECEPTIONIST got fired after submitting her two weeks notice) that the real reason why I didn't get the job at MedXM/ Quest Diagonostics is because " looked mean". I am beyond baffled! I was professional, and punctual. I felt I was more than qualified for the position in which I applied for. These people had no right to turn down my application based on how I looked. That is discrimination at it's best.
This needs to be investigated immediately, because there is no telling how many other potential applicants these people turned down based on their looks.
The only reason why the receptionist opened up about why I initially did not get hired, is because when she submitted her two weeks notice, she was fired a couple days later, and told that she would get "paid off" for the days she missed.
The leadership in this company is just awful. Why have your employees refer someone to a job like this? They not only wasted my time, TWICE, they played with my livelihood. There are too many people out here who are seeking HONEST employment, and at best, longevity with the company.
I'm simply reporting this issue hoping to shed light on what's really going on. I deeply hope that no one else has experienced a level of dishonesty and discrimination as low as this.
A desirable resolution would be to have everybody involved, fired. Good quality customer service starts with leadership, and if the leadership isn't honest, you can't expect honesty out of your employees/company.
poor customer service/hippa violation
This morning 10/13/18, my 9 year old son and i visited Quest Diagnostic located at 970 North Broadway in Yonkers New York. My son was anxious about getting his blood drawn as many children his age and younger are.We were paired with Deanna one of the phlebotomist on staff.
When we walked in, not only did she not introduce herself but provided no privacy for my son. However, i stated my son was anxious as he was scared of needles and had not have lab drawn for a while. My son began sobbing, screaming, tensing up and pulling away out of fear. Instead of being compassionate and understanding his valid fear, her response was"I have a bad back and i am not going to let you send me to the hospital" as result Deanna was unable to perform the blood draw and we were asked to wait in the waiting room until he calms down.
After 5 minutes we were asked back he continued being fearful. Again no privacy was provided as the other customer across the hall spoke through the open door and addressing my son by name in an attempt to calm him. As a healthcare professional myself. I was appauld that he was offer no privacy as these individuals stared at him from across the hall violating his HIPPA rights.
Again Deanna was more focus on her self instead of being empathetic towards a fearful child. "Mom i am not going to hurt myself or your child so, i don't know what to tell you." In a situation like this it is not what you say but how you said it. I was saddened that this individual whom come in contact with persons of different age groups was so belittling.At that point we left the location and decided to go to another location.
We went to Bronx Boulevard, Bronx NY on the same day where the experience was complete opposite. We met Marlene who was nothing but exceptional. Within a few minutes she was to build a trusting relationship with my son . We were offered privacy and lab drawn in less than 5mins.
Quest Diagnostic needs more employees like Marlene who has the skill set to meet the needs of fearful children like my son without intensifying the fear or suggesting a child is too big to act that way as with what Deanna did.
Thank You Marlene for changing my mind about Quest Diagnostics.
