Arizona Medical Board Physician Lauren Poling, DO

Dr. Lauren Poling (IORA Primary Care, Indian School)
has conducted herself with recalcitrance and extreme indifference toward my care.

I believe that it is likely that Dr. Poling will use COVID-19 caution to justify not seeing me, however she could have in the very least emailed or texted me to follow-up and in response to my health concerns; as her health coaches and Dr. Reza has done. She only contacted me after I received a referral to a cardiologist because of heart beat irregularities and pounding while at rest.

I can submit screenshots and correspondence for supporting documentation.

The following is a letter that I sent to Iora Management to be placed with another physician after Dr. Poling failed to respond or inquire about my request for an EKG in March 2021.

Ms. Darling:
Thank you for your assistance in helping me in the move from Dr. Pohling to the care of another physician. I have discussed my concerns regarding Dr. Pohling's apathetic manner over the past year with Dr. Reza on two occasions.
I currently have several issues that require medical consultation, and will schedule an appointment with Dr. Ramirez as soon as possible. During my consultation with Dr. Al-Khatib he mentioned that the dosage of the AMLODIPINE was increased. I do not know when that happened or why but he wants me to return to the originally prescribed 5 mg. dosage. I believe if poor communication with Dr. Pohling had not been a factor this error would have been caught previously.
With regard to Dr. Pohling, the current online IORA advertisements are the anthesis of my experience; however Dr. Ernesto Reze and Health Coach Jason Braden are comendable in their responsiveness and consideration.
Please enter this grievance into my medical file for possible future reference.

Firstly, my Iora experience began very well, however over the past year advertisements such as the one that follows have made me feel depressed and singled out.

After several attempts to address my problems locally and after filing a grievance letter (attached), I never received a response or apology from Dr. Pohling, which tells me her negligent attitude was intentional.

Between November 23, 2020 - June 3, 2021 the only communication that I have had with Dr. Pohling are two text messages.

I also did not receive a reply to the written complaint sent to Ms. Darling. I followed up my letter with Ms. Darling via phone call and she was extremely brusque and stated "This call is over." in a rude tone of voice. When I asked why, she said that it sounded like a lawsuit. I hope that she recorded our conversation.

I later spoke with Ms. Darling's Director (Diane/no last name given), however it was clearly perfunctory. I was inquiring about how I might escalate my experiences and concerns; however a process of accountability was. I therefore reached out to Iora on Facebook Messenger and was advised to contact your office by the anonymous responder that answers inquiries on that platform. [Those messages have been downloaded. ]

The process of reporting and escalating negligence and ethical problems that I have experienced with Dr. Pohling has fraught with avoidance. I question why there is not a "grievance" form provided and why the website does not include a means of communicating with corporate officers?

The "health coaches (Yered/Lorena/Jason/Tania) have been the most responsive and accommodating. Dr. Ernesto Reza is aware of these difficulties and has also been professional and responsive.

Desired outcome: Investigate for the sake of all under her care, that may not have the resources to advocate for themselves. Dr. Poling should not be entrusted to practice medicine because she cannot be relied upon to administer it without prejudice.

Arizona Medical Board Unethical behaviour

I recently hurt my knee. The injury occurred on Saturday 9/11/2021 and my husband took me to an urgent care where my knee was x-rayed and I was told to follow up with an orthopedic doctor. My daughter-in-law is a nurse and she referred me to Ortho Arizona. I called Monday 9/13 and was able to book an appointment with Dr Brian Shafer for that afternoon. I attended the appointment and saw Dr. Shafer's PA. I was asked if I played professional sports and I said no. I was told that due to my 'advanced age' that surgery was not warranted immediately and I was placed in a brace and referred to get an MRI. Not sure what my 'advanced age' had to do with it as I was at the gym 5-6 days a week, was training for a marathon and was very physically active...
I had brain surgery 10 years ago and now have a shunt in my head so the MRI had to be cleared by my neurosurgeon and coordinated with the availability of an x-ray tech. This took approximately 3 weeks in which I continued to wear the brace when I was not in bed. I saw Dr. Shafer after I had gotten the MRI and he was very indignant, telling me my injury was 'devastating' and I should have started physical therapy already. I explained I had been waiting for the results of the MRI and to talk to him with the results. He stated I needed to start PT immediately, he was not ruling out surgery but I needed to improve my range of motion before that could be considered and he set a follow-up appointment for one month. I began PT immediately and went 2-3 times per week for a month before I saw Dr. Shafer again. I saw Dr. Shafer after one month of PT and he had my MRI results. He stated that the MRI was 'wrong' and I was not a candidate for surgery. He asked if I was wearing the brace and I told him whenever I was out of bed, I had it on. He said I obviously was not compliant since I was not wearing it 24 hours a day and he told me to continue wearing the brace, continue with PT and modify my level of activity and I would be fine. I told him I had a second opinion that afternoon and he stated I could find someone to operate, he would not lose any sleep over it. His manner was abrasive, he was rude, condescending and dismissive. Thankfully I had already scheduled a second opinion and attended that appointment that same afternoon. I was seen by Dr. Anikar Chhabra, I had a disc of my MRI results and we reviewed them together. He examined me, recommended surgery and scheduled it ASAP. I had surgery 11/30/2021 and the images taken during surgery showed that the ACL was completely severed. Donor ligament was used to replace the destroyed ACL and scar tissue was removed. In all my interactions with him, Dr. Chhabra was courteous and professional whereas Dr. Shafer was anything but those things. I will not recommend this practice to anyone and will take any opportunity to explain how bad my interactions with Dr. Shafer were.

Arizona Medical Board Urine test procedures

On 11/13/21 I visited the Momdoc office located at 21300 N. John Wayne Pkwy., Maricopa, AZ 85139 to get my urine tested due to a suspected UTI.
I was given a cup with only my first name handwritten on the outside. I entered the rest room and found another person's urine test on the back of the toilet. I found this unacceptable and dangerous. If 2 people had the same first name it could easily have culminated in a misdiagnosis, not to mention the fact I should not have been shown into a toilet that had another person's urine test still in there. There was no excuse as I was the only person there at the time. Overall, I found the staff extremely unprofessional. After I was shown into a room I was asked to completely disrobe and questioned why I had to do that for a urine test. The assistant said the PA would want to examine me. I was also asked the date of my last menstruation. I am an 84 year old woman so obviously the assistant hadn't taken the time to read my paperwork, or even look at me. When the PA entered she asked a few questions then told me I could get dressed. She prescribed an antibiotic and I gave her the number of my pharmacy and requested that the prescription be phoned in right away, (it was after 2pm and a Saturday). She said she would do that. 2 hours later I called my pharmacy and they did not have the order. I called back the medical office and was kept on hold 10 minutes. When she came back on the phone she said it had been called in. When I called the pharmacy again they had received it 2 minutes before my call. No-one introduced themselves so don't have any names

Desired outcome: To preclude any urine mixups in the future. Cups should include the patient's last name and d.o.b.

Arizona Medical Board Severe lack of professionalism and patient care by a physician and the amount of pain he inflicted

On November 3, 2021, I had an appointment with Dr. Glen Halvorson at the Arizona Pain Specialist office located at the 18555 N 79th Ave, Glendale, Arizona 85308. His license number is 13423. I was referred by my medical doctor to determine if the pain in my left arm was carpel tunnel or a pinched never in my neck. I had already been diagnosed by x-ray and MRI with advanced Cervical Radiculopathy.
Once I entered Dr. Halvorson's treatment room, he never turned around and looked me in the eyes, nor did he even introduce himself to me. He simply said, with his back to me "sit down there." I started to explain my situation to him regarding the severe pain I had down my left arm, leaving my first 3 fingers numb, pins and needles pain, and sometime burning. He responded, "he did not care because he is simply there to run these tests." He then proceeded to tape the electrodes on my hand and attach them to the machine. He never informed me what he was going to be doing, what the machine was for, or what he was looking for. He then applied the electric shock to my hand which I thought was a strong amount of voltage. When I reacted and pulled back, he simply held my hand harder. The next set of electric shocks got stronger and I yelled out "Jesus" and again pulled my hand back. Without even looking up he told me "stop using profanity in front of him." I was shocked. The assistant looked at me and was shocked also. The treatment continued for another 5 minutes where he was applying to much electricity where it was hurting me. He simply continued to hold my hand in place and continued the procedure. The real fun started when he brought out a needle and started repeatedly poking me. I again expressed my pain and the only thing they did was get an alcohol pad and wipe the blood away from my hand. No explanation, no apologies, no remorse! After he was done, I asked him what he discovered, and he told me that it was not his job to tell me and that he would send it over to my referring doctor. When he was done, I could not get out of the treatment room fast enough.
I told him that he was the worst doctor I have ever seen. I proceeded to go back to the front desk and informed the staff of the experience I had just gone through. I told the staff that he was the worst doctor I have ever seen and two of his staff nodded their head and told me they agreed with me. One staff member told me they often get complaints about him. Immediately afterwards, two of the patients that were sitting in the office waiting area and approached me in front of the staff and asked if I was talking about Dr. Halvorson. I replied "yes" and they told me that they also agreed with me. This was heard in front of the entire office staff. I asked to see the Office Manager (Veronica I believe) and she said she would look my concerns and get back to me. No one from the office ever did.
I don't need to tell you that a doctor's oath is that they will "first do no harm." It is included in the Hippocratic Oath. If his own staff is conveying to patients that he is the worst and they have received several complaints about him. When his own patients know the doctor and agree with my allegations, there is a problem that is not being addressed.
Thank you for taking the time to read my concerns and frustrations. I am confident that all you simply need to do is ask any of his staff or review previous complaints and you will discover that this is a serious problem you have with this doctor. I will never go to him again; however I am scared for the next person he sees!

On Tue, Oct 11, 2021 I called YVAP to establish myself as a new patient. It took three seperate phone calls for them to finally get their required 18-page pre-registration paperwork emailed to me. I returned it same day after hours. I phoned them back today to get an initial consult set up. I was told they would not set up an appt until they received all my prior records from my last pain management clinic, who they volunteered was not good about getting records sent. I asked to speak with the practice manager (Adriana). She reaffirmed the practice rule - no appointment until the last pain management provider submits my records.

I'm in great, chronic pain (over 3 yrs). I asked Adriana, "So, unless they send my records, I cannot get an appointment scheduled with your practice?" She said, "Correct." I said, "That is a refusal to provide medical service." She said, "We are not refusing you medical service." I said, "You just told me you will not make an appt for me if you don't get my records from my last pain management Dr."

I then told Adriana that I stopped seeing my last pain management Dr because he wanted to perform a procedure on me that he had just become certified to preform, and I wasn't ready for it, was advised by other drs that I should put off, and I feel he then was not careful in my subsequent treatment. So, I was looking for a new provider and was told Dr. Curry was good.

Due to their telling me that my former pain management provider was not good about sending records, combined with the reason I stopped seeing him AND the fact that I am now in a lot of pain, I was intent on getting in with Dr. Curry asap.

I told Ariana that it made no sense to prevent a new patient from at least consulting with the Dr to begin formulating a game plan before some of my records came in. I told her they were essentially holding me hostage by not offering me care because of some arbitrary requirement that is out of my control.

It seems outside of the hippocratic oath to do no harm, which refusing medical treatment does. Whether the Dr receives any of my previous pain management records dating back as far as 2018, as a medical doctor, he should be able to consult with a new patient and move forward with a well-formulated treatment plan.

Arizona Medical Board Desert Sky Spine and Pain Clinic

I would like an investigation into the pain clinic, Desert Sky Spine and Pain, owned and operated occasionally by Thomas Coury, D.O. I underwent many tests and many discussions at this place, all while in incredible knee pain. Dr. Coury is the owner and he performed injections on my knee without proper covid restrictions in place. His behavior was an outrage. Why have himself and at least four other men in the room? He spoke to them only, during the procedure, about such odd things as a new, synthetic form of Valium (I felt he was showing off about his knowledge although synthetic Valium does not exist). He spoke to me only once, when I gasped, and said, "Mind over matter." I was uncomfortable and humiliated. I am a retired dental hygienist and could not imagine what this man, Coury, was thinking. The treatment to myself and my husband by the staff was equally outrageous. His ability to relieve pain to ANY degree is non-existent and his entire demeanor reeks of arrogance. The office is filthy. I had to give a urine sample and therefore had to use the restroom. It was filthy. The furniture in the reception area is decades old (and paint splattered which looks like spilled milk) and allows for no real distancing in the time when covid is alarmingly high in Sierra Vista. I am self-paying, so have lost hundreds of dollars yet have zero pain relief. I have left this clinic with tremendous anxiety and fear, now, of seeking further treatment from any pain clinic in Arizona. Coury's staff is a huge part of the problem as they, too, are exceptionally rude plus, the more frustrated the patient, the more defensive they become. Coury seems to use a nurse practitioner, Maureen Bay, for everything from determining a patient's treatment plan to prescribing medications and she is far from capable, let alone professional at any level. The entire place is, very apparently, there for only money and not at all for helping people. In my opinion, Coury's credentials are not what they should be, having researched him online, and should be investigated to the fullest. The man is a disgrace to the medical profession.

Arizona Medical Board Dr inappropriate behavior

Recently I had a consultation with my doctor, Dr Michael Yim, director at Northwest Health Care 10370 N LaCanada, AZ 85737
After discussing treatment options, we came to the conclusion that we had a difference of opinion, he said then, maybe I should find a different doctor, and I concurred with that option. It was all done in the examination room, in private between Doctor and Patient. About two weeks after, he wrote a letter and sent a copy to Medicare saying that he was no longer going to provide medical care for "inappropriate behavior ", I have no idea what that meant and he also precluded me from seeing any other doctors there. This inappropriate behavior seems to be coming from him not me, he also stated it was in the office, and as we all know exams are in a private room. I have found a different doctor, but I'm perturbed that a professional person such as this can stoop to this level. I've called Medicare to inform them of this false accusation, and I'm writing a letter in response to Dr Yim so he can explain the meaning of inappropriate behavior.

Arizona Medical Board unethical behavior

I went to my Pulmonologist in Yuma, AZ,
Dr. Mallappa Neelappa, MD
For a prescription to get an auto CPAP machine and a home sleep study.
I got the prescription for the CPAP but he refused to give me a prescription
for the home sleep study because he has a sleep clinic and wants to collect the fee.
I previously tried his "mini- motel" and had no success going to sleep so I did a home sleep study with total success.
All I want is the prescription for the home sleep study.

Arizona Medical Board I was verbally and mentally assaulted by the office mgr/husband of my doctor.

To whom it may concern,

My name is Yvonne de Anda [protected]. I'm a 57 yr old, 5'1 woman. My primary doctor is Punya Ramen with Avista medical center in Gilbert, AZ

On July 16.2021 I called my compound pharmacy to see why my estrogen was not filled. They informed me my doctor had NOT approved it. So I call the doctors office and spoke to a woman. In error this woman faxed progesterone over to Strive Pharmacy. I picked up the prescription not knowing it was not correct. Note: The medication looks the same because they make it there. I then had my son's wedding on July 30th and I was on fire! The hot flashes were unbearable! I go to the doctors office in the mean time for bloodwork and waited for results. May I note, I've NEVER had a problem with staff. I then had my tele-med follow up with Dr. Raman. She stated my estrogen was 1/2 of what it should be. Now the unbearable hot flashes made sense. I then retrieved my Strive Pharmacy bottle (All other meds are filled at Fry's pharmacy) and it was PROGESTERONE! Dr. Raman assured me she would fax over the correct med request to Strive. This was on a Friday, strive is open M-F and not on weekends. I was going out of town and called strive first thing Monday and begged for meds next day. The pharmacy pulled up my history and the fax and said the doctors office requested the wrong meds. I then called the office and asked to speak to the lady I spoke to on July 16th. She was very combative and would not admit error at first. She insisted I get both meds at Strive. I implore you to check my history at both pharmacies. After explaining to her if Strive had progesterone requested its because she requested in ERROR! (She eventually acknowledged the error and apologized but not before raising her voice) I then told her that her being so defensive made me feel that it may not have been an accident. She then yelled at me "I'm offended" I then yelled back "I'M OFFENDED THAT I'VE HAD TO LIVE WITH YOUR ERROR FOR A MONTH". I then left a strongly worded voicemail (no profanity) for the office manager. With no response, I left 2 more messages for the doctor directly. What kind of doctors office does not return patience calls? At this point my mental health was not doing well! Knowing this doctor did not care about me as a patient and my mental health for which she prescribes me a generic of Effexor and generic of Seroquel. My last tele-med visit I had told her I had been robbed, so she knew I was fragile, and then to ignore my calls is appalling! Fast forward to yesterday, Tuesday, August 31, 2021. With 2 weeks that has passed and no return call, I have resolved that fact that I will seek another primary doctor. I went to Avista medical and asked to speak with the doctor (I wanted to be sure she knew why I was leaving and also because I needed to get my medical records) I asked several times for my records with no acknowledgement. My luck, it was the lady that sent the wrong prescription at the front desk. When I asked to speak to the doctor and said my name, she very rudely said if I don't have an appointment, I cant see the doctor. It was close to lunch time so I said I would wait. She basically said I'm not going to see the doctor and I said I'll sit here and wait all day if necessary. So I sat down and started watching Netflix. I have to say I am now feeling very anxious. I noticed a man come in with the lady who spoke to me rudely. At the counter he stated very aggressively "CAN I HELP YOU" I took the ear pods out and said "excuse me"? He repeated aggressively "CAN I HELP YOU"! I said yes and who are you? He said the office manager. I said "Great! I've been wanting to speak to you, can we go somewhere in the office"? He said NO! talk to me right here (at the counter) Where are my HIPPA privacy laws? I started to speak, when he abruptly cut me off and said I know the story and you can leave! I'm now in shock and shaking that this man is truly yelling at me! I told him "You won't even listen to my side"? Again yelling at the top of his lungs GET OUT! I called out for Dr. Raman, because now I'm in full mental break down, I'm shaking and felt so violated at by doctors husband and the office. Now this man knows my chart and knows what medication I'm on, and clearly used it against me. I said I'm not leaving until I see my doctor. He proceeds to call me names like Psycho, crazy [censored], your mental and I'm shaking so much at this point I'm in defense mode. There was a big [censored] YOU from me. I know the doctor can hear her husband verbally assaulting me and my mental history, I really thought she would come to my rescue and tell her husband he cant talk to patients that way. But the woman who was in charge of my medical needs and my mental well being was nowhere to be found. Dr. Raman's husband then flew around the counter to physically attack me, I planted my feet ready for is attacked and said "put your hands on me, you son of a [censored]! I dare you".
I'm sure you can imagine my terror that this man and I were touching toes. I'm only 5'1 and he knew he was terrorizing me! He said "Look at you! You're crazy! look the cameras are seeing how crazy your are!" I said there going to show you attacking me! Mind you his staff including men witnessed this behavior. He then told the lady to call 911 and I gladly waited until the police arrived. I have been unable to stop crying and I'm now in a very bad place mentally. This man has triggered me and took me back to a very physically abusive time! I'm absolutely devastated that my doctor had no concern for my well being. Isn't that why they take their oath is to protect us patience. I hold Punya Raman responsible for what happened in her practice. Shame on all of them! SHAME

Desired outcome: I want her held accountable!!! She doesn't deserve to be practicing. She doesn't protect her patients from her bully husband/office manager

Arizona Medical Board Unnecessary / excessive medical treatment

To whom it may concern,
This letter is to report Fraud related to unnecessary treatments performed by Dr. Kelly Favata. Also inflating my records with symptoms I didn't have. I went under several ELVTs and Varithena, causing permanent damage to my veins.
In March, 2017 I made a visit to doctor's Favata's office, for minor varicose veins. I had no major pain or swelling, but I had many superficial varicose veins that I wished to be treated.
I didn't know how it could be treated and if the insurance would pick up the payment or not.  I didn't have leg pain at that time, just veins that were superficial, not large, bulging or big. My legs were overall healthy, despite the trivial problems described above.
I don't have any previous medical history of vein disease.  
I saw dr. Favata in 2017, and she prescribed eight EVLT treatments and she said the sclerotherapy would be performed at the end, after the eight treatments were performed.  
Due to my inexperience, I trusted the doctor and followed exactly what she had ordered. I did all eight EVLT, but I don't remember if I had the sclerotherapy at the end the treatment or not, because her schedule was very busy for sclerotherapy.
In October 2020, I started to have persistent leg pain, swelling, and many small veins coming from the back side of my right leg. I started to wonder if the previous treatment from 2017 caused these lower leg pain issues that I have now, because something was very wrong with my legs. 
I had an appointment with a second doctor; Dr. Wickenburg, he did an ultrasound and his technician could not understand why some important veins from my both legs were missing, and also the vein on top of my thigh was missing. Dr. Wickenburg said he would not do anything, besides sclerotherapy on the back of my right leg to easy the symptoms. 
I also made a cash visit with my first doctor, he doesn't accept the Care First anymore, he is the doctor who I first saw in 2007, he was surprised and asked if I still have any veins left. He said the reason for the pain and swelling is that the end of some veins end is open and the blood flows back. I still thinking about coming back to him, but he doesn't accept the insurance I have.
I made an appointment with dr. Favata again, in a pursuit to discover what treatment dr. Favata did, and the name the veins that were treated. I started all over as a new client again. The first visit was with the technician, probably for screening, who took pictures of both legs, asked many questions and made an appointment for an ultrasound about a week later. The second visit was for an ultrasound, with a technician again.  After the ultrasound, I was told they would give me a call if I qualify for the treatment meaning that my insurance would cover it.
A few days later I received a phone call from dr. Favata's office saying that I would qualify for the treatment with VARITHENA on both legs and she urged to make an appointment. Dr. Favata's secretary said the bigger the vein the more foam is needed, I was surprised by great amount of work I was told is needed to be performed, despite all the treatments I had in 2017. I believe she didn't read my previous medical records, and work performed. She made two more appointments for me, one appointment for each leg for Varithena, about a week or two apart.   
I accepted the appointment, so I could have a chance to speak with the dr. Favata to ask about what was done on my legs in 2017, but in any way, I had no intention to go under any additional procedure. The only way to see dr. Favata was in the operating room. I needed to keep my appointment so I could have a chance to talk with dr. Favata, to ask what was done with my legs.
The first appointment was set for On October 8th, 2020, in the operating room, I asked the technician what veins were treated in 2017, the technician told me the names of the veins and I also took some pictures of the chart on the wall showing the veins. I asked for a copy for my records she had on her hands, but she refused and said she had to ask the doctor for permission, but she never gave it to me. 
I wanted to leave, but I was convinced by the technician to stay, the technician said it was a very easy and common procedure with Varithena and she said it is a very easy treatment, much easier than in 2017, treating only the superficial veins, and nothing like the previous treatment performed in 2017. She put the heat on my right leg, and about 45 minutes the doctor came and injected foam on my upper and lower right leg and she quickly left the room. There wasn't any explanation from the doctor Favata about the procedure or the procedure performed in 2017. 
Also, 1 more varithena on October 8th, 2020. I ended up going under the procedure on my right leg, with the foam Varithena, I in two major veins on my right leg. 
I came home traumatized and cried the entire night thinking about possibility of further worsening the symptoms and conditions of my leg, thinking how could I let myself be induced to take a treatment I didn't wanted. They urged me to come there again to do the Varithena on the left leg, with I canceled immediately after filing a petition for my medical records.
After the experiences described above, I realized something is wrong with dr. Favata's office, because of the extended number of the treatments, never see the doctor for a consultation, only see the doctor on the operating room, and my medical records were wrong. I had 8 EVLT treatments in 2017, the extent of the procedures was never explained, the veins involved, these treatments were way more than I ever imagine. The technician just say, these are minor veins. Now I ended up with 2 more Varithena in one leg in 2020, and canceling further Varithena appointments.
I asked for my medical records from Dr. Favata. My medical records were inflated with problems I didn't have.
My medical chart is exaggerated, with swelling (none), edema (none), restless leg (never had it), which I never had.  
I am concerned about the irreparable damage done to my both legs, great amount of important veins on my legs are gone.
I realized the mistake I made because of my lack of experience in trusting this doctor.
I have pain every day, and swelling on my ankles after taking my socks off, and worried tomorrow about potential blood clots and the complications that can arise because a great amount of veins were unnecessarily taken from my legs, besides the persistent pain and other issues. 
Vein issues do no happen overnight, and I didn't have any medical history/records that show extreme veins problems, or doctor visits or need for these extreme treatments performed by Dr Favatta. The treatments performed are as follows:
Treatment # 1. EVLT of the Recanalized Right GSV (great saphenous veins) Mid-thigh.  
Treatment # 2. EVLT of the Right SSV (small saphenous vein).  
Treatment # 3. EVLT of the Right Anterior Duplicate GSV (great saphenous veins).  
Treatment # 4. Medical ECA of Multiple above below knew intrapreneurs tributary veins. Left Leg:  
Treatment # 5. EVLT of the left SSV.  
Treatment # 6. EVLT of the Left AASV (anterior accessory saphenous veins of the thigh).  
Treatment # 7. EVLT of the posterior Duplicate GSV (great saphenous vein).  
Treatment # 8 EVLT of the Posterior Duplicate GSV (great saphenous vein)
In October 2020. Treatment# 9. Great saphenous veins on the right leg and also one more vein below the knee on the same leg.
Treatment # 10. Great saphenous veins on the left leg recommended by doctor Favata, which I refused.
The reason for this letter is to report this incident. Also, I am worried about the consequences of these treatments.

Arizona Medical Board Unprofessional behavior, bullying and name-calling

I had my annual appointment with Arizona Oncology, 1845 W Orange Grove Rd Tucson AZ, at 11:00 AZ June 23, 2021. Dr Jennifer Hallum did the exam. After preliminary questions she then asked me if I had taken the Covid vaccine. My answer: "You will have to put a gun to my head and tie me to the floor before I'll take that shot!" She asked why. "I will not be a guinea pig for an unproven drug and it's my personal choice." During this dialogue she told me I was "irrational" and a "danger" to her and everyone in that office and waiting room. She tried to belittle me and told me people could "die" because of my decision. I mentioned the two drugs used to treat Covid: hydroxychloroquine and ivermectin. She said "they don't work!" That is a blatant lie! I asked her to respect my decision not to take the vaccine and she would nothing to do with that. I have never had any problems here before. Her husband, Dr Alton Hallum, is the surgeon who performed my hysterectomy in 2018. I have never had an issue with him. I am making this complaint because I was completely taken aback by Dr Jennifer Hallum's unprofessionalism and judgemental and condescending attitude towards me. All because of the vaccine. I vented my frustrations to one one of the staff (Anthony) outside. He told me there have been other complaints as well. He did not give details.

Arizona Medical Board Dr. Martin Langford

He is negligent and boders on malpractice. Dr. Langford and his team laughed at me when i told them i was juicing. I wasn't doing chemo radiation and surgery and i was told i couldn't do it anyway, but i didn't want to do it anyway so i am juicing. I am healing. The body can heal itself with the right nutrients. And it is happening. Dr. langford is pissed off cause it is working and would not give me health provider encouragement and positive reinforcement. When i asked him if a 25 % drop in tumor markers would be significant(and i knew it would) he looked at floor and barely shook his head yes so as to not agree with me too much, cause it wasn't chemo, radiation and surgery. The burning was subsiding a great deal under my arm in axilla and i asked him what that would mean and he said it was because something was rubbing against it on outside. The burning was inside of me on the tumor. What he said was stupid. I said no, the tumor was burning and it is not burning anymore. So i said to him, that would mean it is healing, wouldn't it? He slowly nodded his head and looked at the floor, once again not admitting to the truth, only barely. I had to drag it out of him. He was being negligent and malpracticing. First blood test, i asked him what he was looking for on blood test and he showed me ca 27.29 tumor marker section. So i watched tumor marker dropping significantly over 5 months. From 80 first month to 70 second month and then next time there was a 65 % drop. And this last blood test another 6 points which in all is 72 % drop in 5 months. He never once remarked on how well i was doing or the progress i was making and never said anything about significant drop in tumor marker. That last time i went to see him, he did his 10 minute thing and then went to leave. I asked him on his way out, how come he didn't remark on tumor markers dropping a whopping 72%. I had to drag it out of him. He should have said something to me the minute he walked into the room about significant decrease in tumor marker. HE SAID NOTHING. NOTHING. i had another ultrasound later and radiologist said petscan is more accurate. Langford tried to tell me that ultrasound was more accurate than any petscan or hand measure. Langford said that cause ultrasound was showing bigger size and wanted to lie to me so i wouldn't think juicing was working. Ultrasound later on after i fired him showed a signifcant decrease in tumor size, about 2.1 cm. Oh by the way, he killed his wife with chemo, radiation, and surgery. That fraud of a doctor also put in my medical chart that tumor markers dropping, even significantly 72 % doesn't mean anything. Then why the heck was he looking at the blood test ca 27.29 that he showed me in the beginning. He's pissed off that i am healing. I saw him from jan, 21, 2021 to April 19, 2021.

Arizona Medical Board Missed diagnosis

In 2008 I had a hip replacement, performed by Dr. Amber Randall of Flagstaff Bone & Joint. Things were fine until 2017 when I started experiencing sharp pain, inflammation and swelling in the same hip. I returned to Bone & Joint where an x-ray way taken, showing cysts on the bone around the implant. The PA thought this was unusual but did not offer any explanation or prescribe any treatment. I continued to experience the same symptoms on and off over the next two and a half years. I saw Dr. Randall several times over the course of that time (a total of seven appointments in all) but the problem was never resolved. She ordered an MRI at one point but it was inconclusive, as was an ultra sound. She also ordered blood tests to check for traces of metals in my system but these came back negative. In December of 2019, on my seventh visit to Bone & Joint she decided to drain some of the fluid off my hip (the fluid had been there for over two years at this point. The fluid was tested for metals but came back negative. In January of 2021 I switched doctors and went to Dr. Bourk Cashmore at Northern Arizona Orthopaedics after I had fallen. He determined I had suffered a hairline fracture in my hip but he was also concerned that I still had the cysts on my bone and that I still had inflammation and some fluid around my hip so he ordered a CT scan. The scan revealed extensive bone and tissue loss around my hip implant and recommended I have surgery as soon as possible. A blood test a few weeks later revealed a biological infection around my hip implant which was causing the bone and tissue damage. On May 3rd, 2021 I underwent hip surgery which required the complete removal of my hip implant in order to properly treat the infection. A temporary implant was put in and I was discharged three days later. I have had a pic line in my arm since my discharge and I have received an antibiotic through an IV daily since that time. I have been unable to work and can only put 25 lbs. of weight on my left leg. I am scheduled to have a second surgery in August to put a permanent implant in my left hip once the infection has been completely eradicated. My complaint with Dr. Randall and Bone & Joint is that they failed to check for infection during any of my visits to their office and that they never ordered a CT Scan which would have revealed the issues in my hip. I was told by Dr. Cashmore that if this had been detected earlier my surgery and recovery would have been less complicated. I feel that Bone & Joint did not show diligence in this matter in trying to determine what was wrong with my hip. Clearly bone cysts are not normal, nor is it normal to have fluid buildup around the hip joint for over two years. Yet each time they did a test (x-ray, MRI, ultra sound) they would tell me the tests were inconclusive but they didn't suggest any other type of testing (such as a CT scan) until the next time I made an appointment. Then I would leave that appointment still without any answers. I feel that if they had pursued a solution more aggressively I might not have required the surgery in May or the upcoming one in August. While these things cannot be undone I don't think it is unreasonable to request Bone & Joint to help with my medical costs and lost wages from being off work. I gave them numerous opportunities to help with a diagnosis but in the end I had to go to another doctor in order to resolve my hip issues.

Arizona Medical Board Complete lack of medical care from a local primecare facility.

I have come to the conclusion that the city of Yuma Az and, most likely all of Yuma County lack the ability to provide medical care approaching that of most third world countries. It is unbelievable that you cannot even contact a so called medical provider because their landline service is not functioning and they have no other method of making contact. It is a very disappointing and shocking experience. Having come from an area of the United States that 15 years ago had half the population of the city of Yuma and had a cell phone backup system in case there was a landline failure, I find it incomprehensible that a city the size of Yuma apparently has no such system. So, in an extremely important medical situation, being unable to drive, having no extortion contract with Rural Metro, one is left to suffer with pain, nausea, and severe anxiety due to a medication prescribed by a local PrimeCare facility. I had to cancel an MRI appointment because I had no Idea If I would be able to actually go to the appointment. So now, my suffering, including complete lack of sleep, continues.

Arizona Medical Board Dr. Michael Billhymer

I was admitted unconscious to the hospital and Billhymer was on call. He left a rod sticking out of my foot for a month. When I went to finalize the remainder of the surgery I was told they were negotiating with my insurance company because they were out of network. Then when I scheduled another do tor, Billhymer's office called left a recorded message telling me I could cancel the other appt since they were finalizing with my insurance company. I had the surgery in the 90 day umbrella of the first surgery and then ended up being asked to pay for the second surgery out of pocket. Then for follow ups to the second surgery they began demanding I pay up front out of pocket for any visits. I was in a terrible accident and Billhymer's office did accept payments from my insurance but being greedy and awful have continued to mend tens of thousands more due to their own mistakes! Shame on them. I have since found another ortho who basically removed all of Billhymer's implants and had to redo my entire ankle and fuse it..

Arizona Medical Board Quality of care lacking significantly

I was seen for HRT back on 11-17-20 at Desert Sky Spine and Sports Medicine. I was solicited for this service at my place of employment due to the fact that they were opening a new part of their practice for HRT. I went to the appointment for a "free service" for HRT pellet insertion. I was told at the appointment that I would need follow up labs done in 4 weeks. I was also told at the time of the appointment that I had a thyroid disorder called Hashimoto's disease and they recommended changing my medication as it seemed to no longer be working for me. I had to find an endocrinologist as my PCP did not feel comfortable changing my medication. I was also told at the appointment by a NP, Natalie Kollasch that she would be prescribing progesterone for "breast health". After the appointment, I was extremely disappointed. There was NO follow up care, I chased them down to get a lab slip, which was 6 weeks post insertion, not 4 as they originally recommended. No prescription for progesterone was ever sent in. After numerous phone calls and emails to the office manager, Luis Barrera, I was granted a follow up appointment at no charge due to my frustration. I went on the afternoon of 1-21-21 to see NP Ashley Schultz. I was greeted by the front desk staff and handed a list of "pain medication" to review. This irritated me as the list was over 4 years old and used as a one time basis for some broken ribs I had back then. Apparently, they randomly pull medication "lists" from local pharmacies, even though it may be outdated. Then the MA, Christina, took me to an exam room to do vitals and check me in. She asked me.."on a scale of one to ten, how is your pain level"? I told her my irritation level was at a 10, but no pain as I am NOT one of your pain management patients. I have been confused for one of their pain management patients on more than one occasion. Shortly afterwards the NP came in and I expressed my extreme frustration with the whole practice and did not know if I would be returning. She apologized and we spoke about plans for future care. Upon leaving the office, I was given a lab slip for follow up labs and was told she would be following up with me afterwards. She said she would take care of the progesterone RX and call in a prescription for testosterone as I decided I no longer wanted to do pellet insertions. None of this took place I had my labs drawn on 1-25-21 and never heard from Ms. Schultz. I called exactly 1.5 weeks after labs were drawn and started "bothering" the office again, Again, no follow up care. I spoke with Ariana, another office manager on 2-16-21 and she said that I basically was no longer welcome there as they could not "give me what I needed". I guess adequate patient care is NOT something they could provide> If they can't keep their patients straight and don't know what they are doing, they should NOT be doing HRT. Today, I received a letter from the NP stating that they are "withdrawing from further management of my medical care due to conflict of interest". This is not my first HRT provider and I feel like they just used me to fill a quota they had to become new HRT providers and then just left me to twist in the wind, hormonally unbalanced! They should NOT be doing HRT!

Desired outcome: An apology for makimng me feel like I was the problem, when in fact they were mismanaging my care. I also think a formal medical board complaint needs to be on file as patients must be warned

Arizona Medical Board Banner Thunderbird Medical Center

My son went to Banner MC scared had covid 19 had cough and fever and was scared. Why would they prescribe a person with covid 19 antibiotics and oral prednisone which will lower your immune system. He passed away the covid took over. The hospitals should not give them drugs when they have covid and kill them . Looking threw his medical files he was just scared not sick of anything but affects from the covid. But info on covid 19 and not be scared
stay calm, stay home it will pass.

Desired outcome: Have sheets of paper with info. on covid 19 to give them. Take your vitamins eat well it will pass soon

Arizona Medical Board Lodge at sedona

It distresses me to even have to write this letter and it is a sad commentary on the Sedona Lodge that they have refused to work with me to resolve this issue. On Thursday October 1, 2020 I contacted the Lodge at Sedona about attending their water fasting retreat center. At the time I contacted them, I informed them that I have serious medical and mental health issues - Ehlers Danlos Syndrome and active bulimia and anorexia. I was worried about the impact fasting might have on me and my ability to do it given my medical conditions. The person I spoke with informed me that their treatment would be highly effective for my medical conditions. I have since learned that offering medical advice to ill people sight-unseen over the phone is a breach of medical and professional ethics. Anyway, I expressed my hesitation to go to the Lodge but also my desire to do anything to help my health and at that point the person I spoke with advised that if I wanted to secure a low-price room rate I should give my deposit over the phone. I asked what would happen if my doctors and/or treatment professionals told me fasting was bad for me given the eating disorder and they told me that they would of course refund my deposit. He told me this several times and I actually called several times over the course of a week to confirm prior to putting down a deposit. He processed my card payment over the phone without me ever obtaining paperwork or a contract until after processing my card payment and what he told me prior to processing my credit card payment - that I could cancel at any time - did not correspond with the documents he sent AFTER charging my card $2, 379.93. He stressed that it would be easy to cancel and get a refund but that, if I wanted to obtain a discounted rate, I needed to "pay now." Obviously, this was nothing more than a sales tactic and very manipulative but I had no way of knowing this since I did not receive documentation until after he took my money. I am including my telephone bill from October where you can see an 18 minute phone call at 9:19 am on October 1st during which he took my card payment over the phone. I received the email payment confirmation at 9:45 am and this was the FIRST time they stated that deposits were non-refundable and this was in direct conflict with their prior verbal promises. You will also see evidence of a phone call on 9/29/2020 and numerous phone calls the week before they processed the payment because I wanted to ensure that, prior to paying them, they knew my medical status. I had informed them that I did not have a doctors' appointment for several weeks, but they advised that it would be better to put down a deposit and secure the current rate and that I could still consult my doctor to see if it would be counter-indicated to water fast and let them know afterwards if I was able to come. They stated that I would absolutely be able to cancel if my doctor informed me that fasting was not advisable. It was after they said that and they processed my payment that I got the paperwork they sent stating that all deposits are non-refundable. That paperwork was sent at 9:45 am and is included in the documents provided. I contacted the Lodge again because their email contradicted what they had told me MANY times on the phone and again I was informed that they would allow me to cancel if my doctors objected to water fasting based on my medical conditions. After that, I spoke to my therapist about my desire to attend and she HIGHLY recommended that I cancel. She advised that attempting to fast could kill someone with my medical issues and I then contacted the Lodge and told them I couldn't come as planned. They canceled my reservation at that time and did not put in a future date for a future reservation. I notified them well within the cancellation range parameters that I would not be coming and I spoke to two people at that time, both the front desk person and a doctor on staff. They cancelled the reservation but advised me to wait to cancel completely and process the refund until after I spoke to my medical doctors. In the meantime, they converted my reservation to a gift card so that the money would remain on my account. I want to point out that I contacted them in mid-October, a MONTH ahead of my initial November 16 reservation, because I wanted to ensure that I was well within the 30 day cancellation window. They told me it was no problem to cancel my reservation, which they did over the phone, and they said that I should seek a doctor's opinion prior to deciding to get a refund altogether. They assured me, again, that they would defer to my doctor's recommendations and even made a special point of reminding me about the lab work they require for people to embark on a water fast. I did what they suggested and a few weeks later I went to see a doctor, taking Sedona's blood testing forms with me, and the doctor told me that fasting could kill me or severely exacerbate my health issues and there was no way I should go. He advised me to cancel immediately. After that, I followed up with yet another doctor who told me the same thing and told me it would be medically irresponsible and negligent for anyone to suggest someone with my history would be helped by fasting. I even contacted True North, another fasting center, and the doctor there told me that given my history he thought fasting would be severely damaging to me and that under no circumstances should I do it, now or in the future. Following all of this data-gathering, I contacted the Lodge at Sedona and spoke to two very kind front desk people both of whom suggested that, given my history, a refund would not be a problem and also considering the promises that were previously made to me, they would be happy to help. They then transferred me to the medical director whose behavior was deplorable! He told me he would not give a refund despite the fact that fasting could kill me. I reminded him that I gave my deposit over the phone and was not made aware of the cancellation policy until after the card payment had been processed and I begged him to issue a refund so I can utilize the improperly obtained funds to seek proper care/treatment for the ED. I told him what my doctors and therapist and the director of True North had told me about the dangerous consequences to fasting. I pleaded with the doctor and told him that an attempt to fast could outright kill me and he basically said "So what, that's your problem?" He was cruel to the point of being heartless. I did NOT at any time receive ANY services. I was lied to and misled and they are attempting to steal money from me. Nothing about their behavior is conscionable. I am including along with this letter proof from my phone bill that the deposit was placed prior to them emailing me anything saying they had a no-refunds policy and 2 doctors' letters as well as several online articles demonstrating the dangers of fasting to individuals with eating disorders. I am also including information about Ehler's Danlos Syndrome, the connective tissue disorder that causes me severe GI distress and joint problems. My doctors also advise that fasting would be bad for EDS and could trigger terrible symptoms and lifelong ramifications. The Lodge at Sedona should have known this or, at the very least, should have stood by their promise to honor the medical and mental health advice of my doctors! They made false promises, dispensed incorrect medical advice, and have not provided any services to me at all. I really hope this organization goes out of business as their behavior is putting people's lives at risk.

Note: I tried to upload my evidence but the images were too large. I am HAPPY to send all of it in via email or any other means but I have a LOT, especially convincing are the two doctors' notes.

Desired outcome: I would like them to do as promised and refund the $2379.93 to me

Arizona Medical Board Dr. Kristy Rae ingebo gross misconduct

This doctors behavior is unethical and very disturbing to me that she is allowed to practice medicine around anyone she not only lied on medical documentation she also lied to Detective Britt to obtain criminal charges with on me, she exasperated my sons newly diagnosed condition (By Dr. Hussam mullah) by enforcing a no food and drink order. Drugged up my son so when officer Cobbley came into the hospital to take pictures that is what the officer saw. It is evident my son was held down and drugged before the officer came to take pictures or my son would have been screaming to go home. Her actions have ruined my sons life and mine. My son she said ruined my life as if she could speak for me. I have audio of her pressing for my prosecution and for jail time so she could tell my son Jonathon he was never coming home I have the records pictures everything to prove what I am saying is a fact. She should be permanently stripped of her medical license and never be able to do this to another innocent family ever again! Attached is the exasperated condition that this doctor and medical staff inflicted on my son and the pain they put my son through and myself I would send audio files but would have to email them to you my email is [protected]@gmail.com phone number [protected] this woman abused my son and she should be held accountable for it.
