Physical therapy of me.
The PT treating me over 2 mos. during July & Aug. 2023, when I confronted him of learning AFTER 6 separate treatment sessions of dry needling that $50 additional charges for each dry needling session totaling $300 was charged to me, HE said I know & would be more if was not for Medicare. Had I been made aware I would have stopped Prevea PT & went back to ATI PT where they do not charge their paients for dry needling. The reason I went to this particular PT is that he was suggested by a Prevea DR. having over 20 yrs. as a PT. Seems decieving that no one at Prevea alerted me at any time & I should not be charged $300(waived) & if I have to pay this week be reimbursed. I paid ALL my copays at the time of each session. A need for being transparent with your patients!
Claimed loss: $300 as stated above
Desired outcome: $ 300 charge waved/reimbursed as stated above

Billing
I have been charged with two general yearly exams for my children. I had active insurance at the time of service. They failed to "scan" my card properly at the time of service. I have spent hours arguing with the staff and have been since put in collections all because the receptionist failed to do her job one afternoon. My coverage changed the next year and I did not have my old insurance card from 2019 in 2020. This has continued to make them claim I did not cover my children even though they have always been covered. This has caused me hours of duress as well as the fact they are ruining my credit over a matter that should have never had even happened. The staff is utterly incompetent.
Desired outcome: I have a claims coordinator helping me through my insurance
Care received and billing
I had a high risk pregnancy, I moved to Green Bay from Minneapolis where I has a full care team. I had low platelets and a blood cloting disorder. The hematologist in Green Bay looked at my case and said he did not need to see me again. My OB doctor asked me if I wanted to continue to monitor my platelets, the fetal specialist told me she didn't need to see me again. The doctors here do not care about their patient they phone in their care. I was so upset I started doing virtual visits with my Minneapolis doctors. My blood cloting disorder and low platelets are a big issue, my doctors should have donw better! Now I have over $4000 in medical bills for awful care and they don't offer any financial assistance! This place is a joke and are only in it for the money. I am fully aware there is a pandemic and health care providers are worn out, but my other health team did a great job at making me comfortable and didn't brush off my concerns.

Doctor Mildenberg: Prevea Clinic Howard
I am a 65 year old male who had a yearly physical on 1/6/21 with Dr. Mildenberg. It was my first (and last) physical with him as I had been a patient of Dr. Strungs (a phenomenal doctor). Prior to my exam I mentioned a shortness of breath and wheezing concern. During the exam the doctor neglected to even check my breathing until after he was done and I asked him if he was going to... this has always been a standard procedure for a physical and I was shocked that he would overlook the heart and lungs. He also failed to have me undress and did not test my testes. It was a very substandard exam and at my age this is quite concerning.
Today I was turned away from Biolife where I give plasma because of a letter Dr. Mildenberg had sent over due to their need for a yearly update on sleep apnea. The issue had nothing to do with my sleep apnea, but rather unrelated issues which I feel violated my HIPPA rights. They told me they had never received so much information and assumed my entire chart had been sent including information that Dr. Mildenberg had not been a part of.
I am extremely upset with his unprofessionalism both in the exam room as well as in sending information I had not signed a waiver to be disclosed.
Respectfully:
George Wood

Desired outcome: No contact by the doctor but a follow-up with someone in authority there at Prevea.
Addendum to complaint made 3/9/21... In paragraph 2: the line that starts with "The issue had nothing..." should read: The issue Bio-life had which caused me to be turned away was a failure by Dr. Mildenberg to write a conclusion/diagnosis for my wheezing/shortness of breath which had nothing to do with my sleep apnea and has since cleared up.
Thank You:
George Wood

your billing practices
I was required to assign a PCP this year for my insurance. I chose Dr. Cupino in Lena as his is the office we have been going to for around 20 years. He is the only MD in the office and as such is frequently unavailable. His PA Janet is a warm, kind, funny and knowledgeable person and honestly, we prefer seeing her. However, since I can only have a PCP that is accredited through UHC as a T1 provider and I frequently can't get in with Dr. Cupino, we are now being assessed charges applying to our deductible for using a non-tiered provider. When I contacted your billing, I was told it wasn't something they could fix. If I saw Janet, they have to bill under Janet's name. I know that's not always true as I work for an insurance company and I'm asking you to consider submitting the charges under locum tenens for Shawn Hyde and Carra Hyde for September, October and November.
I do not expect you to require Dr. Cupino to work ridiculous hours, but if I'm seeing his PA because he's unavailable, I expect to be able to apply my benefits appropriately. It's not the doctor's fault he's booked and it's not ours that we needed to be seen at the times we were seen.
Here are 2 of the account numbers for reference. 634900 and 640514. If you need to contact me, my phone number is [protected]. I have a confidential vmail. Thank you.
healthcare (or lack thereof)
They do nothing but waste your time and money. I saw Dr. Dana Carroll on 2 separate occasions. The first time, I came in stating that I was coughing so bad that I was vomiting and it was very hard to breathe and I was having severe back pain. I was in & out in under 10 minutes. She told me I'm probably allergic to my cats. Turns out, I paid 2 out of network providers to tell me I was having severe asthma attacks and I had 3 dislocated ribs because of it.
Second time, I was confident I had an ovarian cyst but she insisted I had appendicitis. I had no other symptoms of appendicitis and KNEW that's not what it was.. My blood and urine tests even confirmed that. She suckered me into thinking that a CT scan would show the ovarian cyst if it were present so I went ahead with the CT scan. She called me (aside from the CT scan, a $700 phone call - AFTER insurance!) to tell me everything looked healthy so it was an "unexplained pain". As it turns out, I again paid another out of network provider perform an ultrasound and tell me it was, in fact, a large ovarian cyst that had ruptured. My out of network doctor got to see that the CT scan was never even done on my pelvis so there's no way she could even see that even after she told me she could. She said I had a healthy uterus and ovaries, but those 2 organs were never present on the CT scan. Weird. I will never go back to another Prevea provider, even considering I hit my deductible for the year. I'll happily go to another hospital to receive real medical care when needed. I've filed a complaint with the State of Wisconsin as well.
Refusal
My son has been sick and hasn't been in school for over two months. He has stomache pains, headaches and vomits at least 5-8 times a day. We have seen his doctors and they have referred him to a Neurologist before he sees a Pediatric Gastro specialist from The Childrens Hospital (Milwaukee) but here (near where we live) in DePere, WI. Prevea Health Network employes the only Pediatric Neurologist available but will not except my child because of my past ability to pay previous bills. I had been "termed", as they put it.These bills were excused through bankruptcy in 2007. I had become disabled in 2000 and my medical bills had piled up thus forcing me to file. None of that information was necessary to explain because it is the fact that a medical facility would refuse care at all is the point and a suffering child at that. My disgust overwhelms me.

I have had it with Prevea. I have blood test done very regularly. I have had them for years. One of the tests is done for my kidney and pancreas transplant that I had many years ago. Prevea doesn't do this test and sends it to Mayo clinic to be processed. Not my fault. What my insurance company needs is an order for this test to be done from my primary care doctor. The nurses decide to make a decision on their own and won't authorize it to be done. So I keep getting bills for $257. I am fed up. I called Dr Hoppe today and specified that I needed him to call me. We'll see if that happens. This is ridiculous. If this isn't taken care of and I have to pay this bill, I will be going to another clinic, either Bellin or Aurora.
Just doesn't care anymore
I had a routine pap and pelvic exam. I have had these exams for years and never had to pay anything for any labs or exam fees. When I got a bill for almost $400 in October I thought this was rather odd. I contacted Prevea and they said that I had had an irregular pap and that they had done additional testing. I asked the billing specialist why I wasn't notified. Stephanie, the billing specialist told me that it is required that they contact the patient before doing any additional testing and that she would look into it. Stephanie called me back a week later and told me that since they had found irregularities that they had been justified in doing the additional testing. I asked her AGAIN why I wasn't notified and she ignored my question. She said the charges will stand even though it is their policy to notify the patient before any additional labs are done. I do not feel that these labs should have been done without both me and my insurance company being notified. I will be pursuing this complaint and will be contacting local newspapers, better business bureau and anyone else who will listen.

What has happened to Prevea. Time to switch doctors. They never contact me either. I was told to post questions on my chart and they would answer them but sometimes not for days. I am very disgusted with Prevea. They call and tell me I am going to bleed to death but then when the test is not normal, they don't call and tell a person. The test results are on my chart but no one gets back to me when the test results are not normal. UGH
