Subject: Dispute of bill dated 1/08/2024
To Whom It May Concern,
My initial visit to Great Expression in Snellville in November 2023 I stated that I wanted to use my 2023 dental benefits of $2,000.00 by December 31, 2023. The agent stated that was possible then I chose to get a lower partial plate, cleaning and some filings. However, it was never stated that the lower partial plate must be received by the end of December. I paid $62.00 on 11/28/23 and paid $349.00 on 11/29/23. Therefore, I received my partial plate on January 08, 2024, and then I was billed $957.00. My spouse Tomalene Hanson contacted the office on various occasions inquiring about the charge and initially no one could explain what the charge was for. It wasn’t until around May 2024 I was told it was for the lower partial plate because it was received in January 2024. After that I spoke to Adonna on 5/14/24 and requested an appointment with the office manager Alicia and she set it up for 5/15/204 at 9:00 am. I went to the appointment, but Alicia was not there and wasn’t coming into this office that day and I asked why they didn’t call and advise me, they stated they didn’t know in time. If the office had informed my husband James Hanson that the insurance company would not cover the charges of his lower partial plate until they had the order in and delivered to him, he could have waited until January 2024 to do the work.
On June 10, 2024, Humana made a three-way call and spoke to Beth in the Snellville office and explained the claim was not paid because of the January 2024 date and the claim should be resubmitted with a corrected date of November 29, 2023 and they would still pay a portion of the amount of $957.00. I have called the corporate office at 248-213-1100 several times and left voice messages but no one returned my call. I am looking for someone to assist with this charge as we would really like to get this resolved. Please call Humana at 800-457-4708 so they can explain how to get this situation resolved. Please call Tomalene Hanson at 404-429-9391 or email [protected]@yahoo.com an advise when this matter will be resolved.
Thanks in advance,
James Hanson
