I am submitting this letter as formal written notice under Regulation E (12 CFR §1005) regarding multiple unauthorized electronic fund transfers from my Credit Karma Money checking and savings accounts, which are held by MVB Bank.
1. Description of the Error
An unauthorized individual accessed my Credit Karma Money account and created a virtual debit card without my knowledge or consent. This card was then used to complete fraudulent transactions at a Kroger store in Dublin, Ohio. I reside in Florida and was not in Ohio at any time.
Credit Karma’s fraud team acknowledged several smaller transactions as fraudulent and refunded them. However, the largest unauthorized transaction — $2,705.50 — was incorrectly denied, despite being part of the same fraudulent activity.
2. Details of Unauthorized Transactions
• Total disputed amount: $2,705.50
• Merchant: Kroger

• Method: Unauthorized virtual debit card
• Date(s): Sunday, April 12th, 2026
I did not authorize the creation of this virtual card, nor did I authorize any transactions made with it.
3. Prior Attempts to Resolve
I have already taken the following steps:
• Filed disputes through Credit Karma
• Received approval for several smaller fraudulent transactions
• Had the $2,705.50 claim denied without adequate explanation
• Requested two manager callbacks, neither of which occurred
• Received no provisional credit, despite Regulation E requirements
• Contacted Kroger management, who confirmed they are reviewing security footage
• Filed complaints with the CFPB
• Consulted legal counsel
Despite these efforts, the matter remains unresolved.
4. Request for Immediate Action Under Regulation E
Under 12 CFR §1005.11, I am requesting the following:
1. A full Regulation E investigation into all unauthorized transactions.
2. Provisional credit for the disputed amount of $2,705.50 if the investigation will exceed 10 business days.
3. Written confirmation of the investigation, including findings and final resolution.
4. A copy of any documentation or rationale used to deny my prior claim.
This letter constitutes official written notice, which triggers the investigation timelines required by federal law.

• Proof of my physical location in Florida
