Disorganized, unapologetic, unprofessional staff
I went to get new hire paperwork. I was waiting for 5 hours! Absolutely ridiculous. I remained calm b/c I had to get it done. People on front line seem very inefficient . I checked out and I was told when to have the ppd read and signed paper, and when to return for my part 2 PPD. I arrived for my part2 and no one could find my papers. They had kept all my papers from the employer and they did not have it. Then the girl argued with me that they never have to do two part ppd, and they did not have any of my stuff in the computer. I called the HR department and they told me to leave, and not to waste my time.
I hope more companies see this message and stop using Concentra for services . There are better professional places you can send your New Hires. This place and the way they handle paperwork is a joke and waste of time.
Desired outcome: Get a full new front staff and/ train on customer services and reading paperwork, and give copies to all patients so there is no chance of misplaced stuff, which wastes a lot of time.
Concentra healthcare services
I went in to a local Concentra location for a random drug screening on Fri October 14 2022. Shortly after I had completed the paperwork they wanted me to fill out the lady at the desk called me to the counter and informed me that she had "accidently" given all my personal information to another patient. To my knowledge, that is a major HIPAA violation. Now there is a complete stranger out there with my name, address, phone number, medical information and last but certainly not least... my social security number. Complete and total negligence and incompetence and somebody needs to correct this. Concentra location: 2322 S. Academy Blvd. Colorado Springs, CO. 80916
Desired outcome: Guilty parties need to be fired at the very least. Company should be handed a lawsuit.
A horrible visit at concentra
On 10/11/2022 my Family and I came in for their sports physicals at 9:05 am when I told the receptionist the service I needed for my children she proceeded to throw her head back and roll her eyes and rudely said “It will be an hour to half and hour wait and you will have to pay 35.00 a piece! And then slammed her pen down and proceeded to go to the back , were she spoke to another coworker out loud “Oh and heads up you have three sport physicals back to back!” And the other Coworker stated nasty “are you freakin serious! And rolled her eyes at me. I have never been treated this way in my life. This is the one in Lexington ky Hamburg area proceed with caution.
Desired outcome: I spoke with a manager she said she will handle it then the receptionist came back up front talking about the incident with another coworker, I have never seen such unprofessional people in my life.
Lab results
Hi, I had a needle [censored] after administering a shot to the patient last week on Thursday (09/08/22) I was at Concentra Scottsdale location at 2pm and it took them 4 hours to get by blood drawn and they asked me to a follow up appointment next week and offered me Tuesday since I am working whole day Tuesday I requested for Wednesday. I called on Tuesday to see if i got my labs back. I was put on hold for 40 minutes because there was a patient a head to be taken care and i was told that they haven't got the results back from lab yet. (Aura)is the one who i spoke to then. I was asked to call back later in the evening and i called and spoke to Stacey and she checked and same answer. I had a family emergency and had to reschedule the appt which i did for Friday. But I wanted to see if i got my results and called every single day at least twice a day to see if i got the results back. Today is Friday and i had appt at 3:30pm and i called 3 times since morning to find out if i got the labs back. Now Stacey who is spoke to tells me that she is going to call the lab and see what the status is. People there are so incompetent. I have been dealing with lot of patience and I think there should be some sense of urgency in these matters. I was told that they are going to call me back but never heard back from them. When i asked them they told me it usually takes up to 7 days to get the lab results back. I have never heard Labs taking that long to give the results back. If they know it would take that long why would they offer me for appointment in 4 days.
Desired outcome: I would appreciate if there is some sense of urgency in sensitive matters like these and proper communication. I would appreciate if some one can get back to me as soon as possible.
Customer service-staff trying to bully and engage in violence-falsifying my test
on September 1st, 2022 I went to Concentra to take an pre employment drug screen, I was met at the desk with obstinance and people with lackadaisical attitudes,they instruct you to "grab a clipboard come back when you're done" that is the extent of the direcetions they don't explain they don't overall care, they are their to put in the 8 hours and go home it's very obvious.
The office was EXTREMELY cold, I mentioned that and the 2 individuals behind desk start whispering and giggling and after 40 minutes I walked and asked how much longer and was told to wait they would get to me
I was then called back by a very vey rude disrespectful individual I don't know what her deal was I completed the UA she looks at it and then she puts an temperature strip over the cup over the original one, puts the test strip in the cup I am certain it came back clean I think she was surprised because I am heavily tattooed, and by this time I had been there over an hour I had had enough and let the staff know it! anyways she looks at me and says I can't send this to the lab it doesn't meet certain criteria. ok what is that criteria she would respond I said you know what I knew while watching people come in and out before and after me that those witches at the front desk were up to something I am going to call my employer and I grabbed my stuff out of the basket and turned around to head down the hallway that individual gets in my face almost nose to nose and proceeds to tell me don't talk about my front desk like that I told her to get out of my face and I Went to step around her and she stepped in front of my face again then I went to walk the other way because inside me I was about to snap on her and those type of people aren't worth however she stepped around in front of my face again thankfully by this time I was at the door I told her MOVE and I opened the door to the lobby and as I was walking to the exit door she yells at me and "DO NOT EVER COME BACK HERE AGAIN" wait what did you just do, you acted aggressively towards me, you continuously tried to lure me into a physical altercation which I am self defense trained black belt in TaeKwondo and won tournaments in sparring so I don't think that was a brilliant move, and then furthermore you humiliate me in front of a waiting room with other clients in it what the hell is a matter with her and why is she allowed to work people 10 years ago I was a much different person that would have had very serious consequences today I rise above people that think they are better than, lady yes I looked rough yesterday I am battling cancer so it was a good day for me and you in all your infinite wisdom decided to treat me like some addict off the street which you don't have a right to judge anyone or treat anyone the way you treated me and I can only imagine how many times this has been done but it is my goal to make sure it is the last time it is done by her.
I am 52 years old non addict non drinker and yet she acted like I had so much to hide and I was such a liar I had 3 more days to take the test if I were concerned I would have waited and there was nothing wrong with my test it came out of my body 2 minutes before i handed it to you so that was a blatant lie. and you violated the law by tampering with my sample you had NO RIGHT to put a temperature label over the one that initially came with the cup and please say you didn't I record all drug tests with my phone I turn it on as I begin the process because I have heard of others being treated in this manner at your establishment no doubt well guess luck your luck ran out because this time Ms. whoever you are on video your attitude your demeanor your aggressive behavior me asking you to move and you getting in my face not once not twice but 3 times, this time your tyranny will stop.

William Curry MD Concentra in Salt Lake City, Utah
This so called Doctor William Curry is the most arrogant narcissistic thinks he’s entitled that I’ve ever met in my 54 years but I’ve gone to doctors. He treats his patients like crap and makes you feel like you’re faking your injury he will tell you that the company can fire you within six weeks if you’re not back to regular work by that time whether you’re better or not. I have a tendon or tendinitis in my hand and wrist from doing repetitive work for three years. He told me and I quote “after three years of the same work, your wrist and tendon should be conditioned enough where you shouldn’t have any problems with it” end of quote. Are you kidding me, did he get his medical license in a cracker jack box. Doing repetitive work is what causes tendinitis it doesn’t condition your tendon so it doesn’t happen again. Best advice I can give you is DON’T GO TO HIM. Go to a different concentra.
Concentra medical labs
I was hired by s&c electric I was required to take a physical at concenta medical 614 w Monroe Chicago I took the drug test July 8 they informed me that I fail the drug test due to them finding marinol in my system a drug used for chemotherapy which shows THC result I haven't used any drugs period I requested another screening test they refused they wanted 150.00 money order to reanalysis of original drug screen I lost my chance to start at s& c due too this failed drug screen please can someone help me with this

My new employer arranged for me to get a pre employment screening at Concentra on Capitol Heights Md. My appointment was scheduled for 9:00 am I arrived and checked in at 8:36 am. The staff never gave me the paper work that I had to fill out until I went back to the front desk to ask if I was supposed to wait in my car after the security guard instructed me to wait in my car. I was then asked if I had brought prior authorization from my employer with me which I hadn’t.. The wait time to be seen is utterly ridiculous. If I have to choose I will never come back to this facility again. Also please train your staff in all areas.
Desired outcome: I will be making a suggestion that my new employer Amtrak find an alternative to seen employees because this place is terrible.
Customer service / secretary
I'm currently attempting to call the location in Morrow, GA on Mount Zion Blvd and I was placed on hold until the phone hung up. I've called six times now and the phone just continues to ring until the line hangs up. One time I could hear someone pick up the receiver on the other end and hang the phone up. This is extremely unprofessional and if the office doesn't want to take phone calls for simple, 30 sec Q & A's then they shouldn't even be accessible.
If someone could let me know if this location accepts walk-in drug screenings for employers, that would be greatly appreciated.
Desired outcome: Can email them my epassport instead of printing and bringing it. I went to a Concentra location before that followed that protocol. I don't know if all facilities are the same. Can I walk-in for a drug screening?
Medical Care
This is supposed to be an industrial care clinic .. there were 18 people waiting when I got here to get my arm looked at. 3 receptionists , they said a 2.5 hour wait. I said okay to wait, I went to eat and came back in 1.45 hours. All the same people were waiting and they all went on lunch. People came in with scheduled appointments and were given the same wait time as a walk in. Okay he time I got back I waited for 30 minutes and they barely called back 1 person. Why are they opened if they probably have no provider here? Hey are they charging companies money to see thei employees when they are not sufficiently staffed with any Doctors or staff. This company needs to be reviewed and if owned by Humana or hey need to stop robbing companies for services they do not provide
Desired outcome: Close down this facility or get medical doctors and medical assistants have to see patients
Blood Draw
I went into Concentra @ Westport location (St.Louis, MO) had a blood draw done, On Wednesday the following week I received a phone call that I needed to return to Concentra (same location) due to lack of molecules / cells in blood draw. When I arrive, I stated I was there to have another blood draw. I then hear the employees and Supervisor behind counter annunciating I was there due to lack of cells and molecules in my blood (which is a HIPPA violation). The six people in the waiting area DID NOT need to hear my business. I was told by the phlebotomist that she would put a RUSH on the blood draw which results should be back within 48 hours. The following day I received a call from Concentra (same location) that I needed to come back due to the phlebotomist not drawing enough blood. I said "hell no" I done got stuck twice by you people and that is not my problem. I have no idea what this location is doing with my blood. I received NO paperwork (which I should have received the back copy of the perforated one they fill out). I told that girl if I have to do another blood stick you people are going to pay me for my blood, this is [censored]! This location is very unprofessional, not enough experience in this location. Avoid this location!
I don't believe its sanitary for the phlebotomist to cut the finger tips of her gloves to stick a patient. Huge safety RedFlag for me. A patient should actually watch you put the blood in a bag to send off. Mine was laying on the desk top of the chair both times as I was told I could leave I was done. Horrible experience for me!
Desired outcome: Hire new people with experience or retrain the ones you hired. Invest in your employees to make them knowledgeable and professional to become ethical individuals. Retrain on HIPPA and confidentiality.

There have been several incidences
where signature from physician was
not completed. Owcp and i have
advised that these signatures are
required. I was referred to a dr.
Kujak for a pm&r on 2/17/2022. Then it
changed to a pr4 with the same dr
kujak.On 3/7/2022 per consultation
with clinic manager and director(np
informed me of how the change was
made on 3/12/2022). I went to this appt.
And found that dr. Kijak could not
assist/treat me due to not being
certified for 6th edition(federal). On
3/12/2022 at a follow-up with np
maqsood, she informed me that she
had no experience with fedral
employees(usps) and i was her only
one. This was her explanation on
sending me 125 mile rt. I asked her
wasn't it part of her job to know
the qualification of the medical
professional she was referring a
patient or me too?" she went on to
say"this is who they wanted you to
see". I'm amazed at the disconnect
for decisions that surrounds my
health care and her lack of
accountability and professionalism.
I am now having to wait for a ime/qme
in order to recieve medication
recommended by a previous referal
(completed on 2/7/2022) by dr. Belfer.
Both of these referal where under
the concentra umbrella, so
knowing the qualification of the
medical professional was not out
of reach. I continue to suffer from
6-10 hours of sleep in a 3 day
timeline, high anxiety and mental
anquish.
On 5/16/2022m i was notified again that
the decision to change from a pm&r
to a p24 was the decision of the
director(this came from
leticia/clinic manager). She also
gave me the directors name at this
time(m. Tenison) while i was in the room with
katherine(pa). Leticia stated that dr.
Kujak was now in the process of
becoming certified for 6th edition,
that the director nor she knew of
her uncertified status. I have asked numerous times (via phone, in person) for concentra's business license. I even called select medical to inquire:concentra stated (1) information not availabe, select medical(1) will have someone contact me and on 5/16/2022 concentra's clinic manager(leticia) informs me, per select medical's legal team, the business license will not be shared with me.
*in these 8 months of being seen by concentra for head injury/light sensitivity, i haven't been sent for a ct scan, mri, vision testing or any other testing. All they do is send you to doctor's under their umbrella for absolutely nothing and then boomerang back for a follow-up and another referal., i was seen by katherine
upoff. This was my first time seeing
her and she confirmed she was new,
after i asked about the long
wait(she stated she was new and
went where they told her too). I
proceeded to end a call with
concentra(making a complaint).Pauphoff, then tells me it's a hipaa
violation talking on the phone. I let
her know that no violation has
occurred, due to the fact that no
identifying health information of
any patient was visible or spoke of,
other than my own. She exibited a
low source of hostility and
behavior of wanting to rush. She
breifly went over dr. Belfers
recommendation and stated she
knew nothing about the medication
suggested (emgality). She stated
she was uncomfortable with the
recommendation and was referring
me to pain management. I asked"why
have me go 150 odd mile to come back
to another referal?" i could
conclude by her facial e pression
that she was not used to answering
patients question and agitated.
Getting up and going the the other
side of the room toward the door,
she turns and says"i'm not stupid",
with this animalistic look on her
face. Where this outburst came
from i don't know. I explain to her
that i didn't believe she was stupid
or she wouldn't be in the position
she was in". With the look of"fight
or flight", she says"you can take
the referal or i'll close your case".
Who is this women and why should
she be unprofessional when talking
about my health. I do beleive, i am
allowed to question any and all
about my health care. She turns and
leaves the room. I leave and go out
to the lobby. I tell the front desk
what happened and ask for her name
while waiting for my work status
and future appointment. I then find
out weeks later that the original
work status i was given changed
without my knowledge. Retaliatory
behavior.I have been seen at concentra for

I came in to one of your establishments for a TB test and a physical on may 3, 2022. I received my TB shot and I was told to come back on Friday may 6, 2022
That way they would be able to review my TB shot and I could receive my physical all at once. I was told to come in before 4:15. As I am waiting this I am sitting in the lobby waiting for my name to be called now I am being told I was suppose to come at 4:15 makes no sense to me I just don't understand. How there associates tell me one thing and now I am being told something else.
I'll find out a big problem and I think they need to be trained better. Last name Crisol took my blood pressure and didn't even have you take my coat off. Very unprofessional someone needs a step in and come check on us establishment.

Concentra md santa fe, nm, couldn't understand the mumbings of an injured employee. (Employee injured on the job - TBI).
Do the genious doc called the "employer." Employer has been ebezzling from the company, told the doc "don't believe a thing he/she told you, he's just making an excuse to sue us!" Doc obediently obeys the supervisor, despite the patient's obvious signs of injury.
Concentra needs a class action law suite filed against them. They are scum wanabe dr's who side for the Employer, foregoing employee injury and illness.

Medical card for CDL
Few yrs ago a very rude concentra employee said I had a hernia and said I was unable to work. I had to find a doctor and wait to be seen.
He checked me out and immediately said you do not have a hernia. To be sure he made an appointment for an ultrasound. That procedure came out negative. Basically missed 3 wks of work without pay, spent my own money for medial services because of an incompetent concentra employee. This happened at a concentra in Phoenix on Thomas rd. EVERYONE YOU TALK TO HAS A HORRPR STORY. ITS LIKE THEY GET A KICKBACK IF THEY KEEP YOU FROM WORKING
Desired outcome: I GO TO A CHIROPRACTOR WHO DOES MED EXAMS FOR CDL DRIVERS. WELL WORTH THE $50
No dot card
I went into the Bellmawr office on 3/14/22. This was to get a DOT physical. I got the physical done. When I went to get my paperwork I was given the copies of everything. I asked where is my card. They said they don't give out cards anymore. I had went in on 3/7/22 to get a physical not knowing when I came back from a workers Comp injury I would need another physical. But on the 7th I was given a card. Janie talked to me in a very condescending way. As a CDL Driver of 21 years you need a card in your wallet. You can get stopped and the computer system is down. The driver has nothing to show. I would like my card Thanks

Medical examiner’s documentation
The past 3 days, I have been visiting Concentra on Polaris (Las Vegas) in order to fill out additional details as required by my company for my paperwork to be DOT compliant.
Also, because they didn’t wish to wait on the doctor that performed the original exam, they created confusion by providing 2 dates without an explanation.
Subsequently, I provided additional details on the paperwork. It was run by the doctor on duty to make sure it was done correctly. The only thing needed was for the original doctor to sign off. I provided my email as well.
Then, the hard copy was left for the doctor to sign and I was told that I’d be contacted by phone and / or email.
When this didn’t happen. I returned to follow up. In short, they couldn’t locate my medical paperwork and I was told they’d follow up after tracking down the items.
I just returned again and the paperwork is gone. Not one person can locate it and they are not able to provide contact information to higher ups to take card of the issue they created.
How does one work with incomplete paperwork?
In all, I have returned 3 times without a solution. Just filled out the paperwork again, but have to wait until Monday (3 more days) to receive an answer.
They always seem to know better even though the DOT states something else. The arrogance and ignorance!

I am a nurse case manager and I manage worker's compensation cases and I call providers offices all over the US and I can tell you by far the Concentra offices no matter what location are HORRIBLE when trying to reach someone. The rings and rings and when it sounds like someone is picking up they disconnect you. This has happened to me in AZ and FL [protected] and many other states. Can you please look into this.
