My name is Michael Perez I have a hearing impairment due to childhood illness I suffer from anxiety due to my hearing loss
working out makes me feel better and My mother is a disabled Veteran who I like to bring to the gym every now and then
with me it helps her and makes her happy. I got the black card so when she visits me, and I visit her I can bring her too the
gym with me but due to the lack of judgement and abuse of power there has been wrongdoing which I hope you can correct.
On Tuesday 24th of January 5:00 pm my account was canceled I was banned from all Plante Fitness without reason,
I was denied the use of my black card access by a white male didn't get his name he had a smug look on his face as
if he enjoyed, hurting and putting people down very sad and upsetting but I did recognize the manager, Vicky who
stood by and joined in on the bad behavior. The whole ordeal lasted approximately 25 to 30 mins it was one of my most
humiliating experiences I ever had everyone at the gym stop to watch them talk. down to me belittle me. I am writing
to you because of an unfair and bias treatment I received at one of your Francises which make you. as a whole look bad,
I am a black & Puerto Rican male am quiet. I keep to myself I don't like to bring attention to myself, am shy and embarrass
easily so I go to the gym to help with some of these issues especially my depression working-out makes me feel
better but today I felt so low so low like am not human I was urging them to please stop I didn't do anything wrong why are
you treating me this way. I will further explain what happened, the reason I was given for being banned from all plant fitness
was that I quit without notice. as I finished my workout, I went downstairs to ask the clerk a Hydro Massage the employee
behind the desk gave me a smug look and said come on" Mike you know you're not allowed in here " loud in front of all the
people at the gym as if I were a vagrant, at this point I was confused didn't have a clue of what he was talking about I said
what do you mean I have a BLACK CARD. the male clerk said in even a louder voice and harsh tone "YOUR NOT
ALLOWED IN HERE " I felt so embarrassed he acted as if I was kicked out of the gym. The day before today 01-24-2023
they made me feel less than a person. The manager came to the front desk and said to me I was banned, and my
membership has been cancelled, I am hard of hearing I asked the manger VICKY can we please step to the side to speak
because I didn't want to be further embarrassed by putting my disability on display for spectators at the gym. I didn't have
clue as to what is happening, Vicky said no and cont. to shout from behind the front desk "YOUR MEMBERSHIP HAS BEEN
cancelled. I asked VICKY why. She said many months ago when I was employed by Plant fitness at the HOWARD BEACH
BRANCH. I stopped coming in I quit working without notifying them that I no longer wish to work at their location.
This is untrue at the time I was dating and living with the assistant manager ASHLEY ROBINSON of the Howard Beach
Branch when I was hired ASHLEY and I have been living together for over a year, both applications have the same single
family home address. I informed ASHLEY CHRISTINA ROBINSON I will not be returning to the Gym to work I will be quitting
once she leaves to her new location Wycoff plant fitness. The reason I quit was that my girlfriend and assistant manager
ASHLEY at the time I witnessed her being very promiscuous flirting with costumers and co-workers. I later discovered she
was unfaithful to me by sleeping with one of her co-workers CREW MEMBER JASON DELREAL who also works at the
HOWARD BEACH location, and I wasn't comfortable working there. I felt it was demoralizing to cont. to work at the location
where my live in girl slept with a coworkers and flirted with many costumers. Due to my Depression and Anxiety, I am not
able to mentally handle the situation or be able to perform my duties under such circumstance. ASHLEY CHRISTINA
ROBINSON got promoted to MANAGER and reassigned to the WYCKOFF, and I quit a Coworker said Vicky was sad
because she was losing two of her best workers meaning Ashely and Myself Michael the same week. In that statement alone
says Vicky was aware that I was leaving my employment with plant fitness Ashley and I decide to go our separate ways and I
asked her to move out. ASHLEY, VICKY (Howard beach manger) and NADA (area manager) are close friends there using
Plant Fitness like their own person business, I feel this is a bias act because her and her click. I broke up with ASHLEY
Since these are ASHLEY'S friends and she wasn't happy I asked her to leave my home where she lived rent free and she
couldn't use me anymore so she has a vendetta against me and is trying to hurt me anyway she can. I called my mother
who usually helps me handle certain stressful situations because I become overwhelm easy, she also pays for account
because its therapy for me to go to the gym and work out it make me feel good about being me and it relaxes me so I can
feel less stress and sleep. Her account was charged 26.00 January 20, 2023 did she lose her money and why. I asked Vicky
to speak with my mother to explain, she refused and said she doesn't have to talk to anyone in HURSH SPITFUL TONE she
said this is between you and me. My mother called the Howard beach gym she to speak to the manager she refused and
laughed with the male co-worker it was the hardest day for me I went home and thought of suicide that's how low they made
me feel they made fun at me and my mother who are both disable. I went to this because of its motto JUDGEMENT FREE.

Hawthorne, CA 90250 on March 28 th, 2023 i was physically assaulted by the black worker & the assistant manager.
Walking into the gym the black worker with the dreadlocks followed me into the locker room grabbing my shirt yelling you need to check in.
Being the business owner of several business touching someone without their consent is the most disrespectful thing you can do. Telling the worker don't put your hands on me, I checked in with the woman working the front desk. The worker responded, I'm gone ban yo [censored] today [censored]. Demanding to talk to the manager on staff, i was verbally assaulted by the assistant manager,
When i let the worker know that i was physically attacked by the black worker on staff, the worker says we banning yo [censored] today [censored].
The derogatory language, the level of disrespect, being attacked by the workers on staff, this is not rite. This is not a judgment free zone, this is the slave
zone this gym is being run by crack smoking workers who physically attack the members, there's several reports of woman being raped, stabbed, beaten by the workers
of planet fitness. My life is to be taken seriously, being the black business owner, owner of several business, i won't settle for being physically abused.

women's locker room
Recently, the women's shower room has been dirty, trash lying in the corner for 3 days, hair in the drains and the handicap shower has had an issue with the hot and cold regulator not working properly. I have been told that the part has been ordered, that was 3 weeks ago! Also, sometimes the paper bag dispensary has been overflowing. I think that when there are girls working, maybe they could check on the locker room once in awhile instead of just standing at the desk. I have been a member for 4 years and I don't recall this being such a problem in the past.

My wife had been trying to cancel the membership since 2015 but the account kept withdraw the membership fee. The facility was close few years ago and I called 2 weeks ago talk with someone and she said I will have refund between 5 to business days. Today I called again and talked with the general manager name Josh. He said the account cancel today and I will get my refund next 10 day. Why it's so hard to cancel the membership since 2015? I belong to Planet Fitness in Dallas. The Webb Chapel location. I shower before work. Therefore I am semi naked or naked. There is a woman cleaning the men’s locker room. I believe this is illegal in Texas. You certainly would not have a man cleaning the women’s locker room. I brought this to the attention of the local management and nothing has changed. Can't reach planet fitness headquarters about my membership issues

On Saturday March 11, 2023, I open up a planet fitness black card membership for my 15-year-old son at the planet fitness on western Ave in Knoxville, Tennessee.
On March 11, 2023, I completed and signed the parent and guardian waiver for my son Rain Fields, he also signed the form and had his photo taken.
On March 12, 2023-April 3, 2023, visited another planet fitness check in denied, called planet fitness on western Ave 4 times they said this problem can’t be fixed.
On March 16, 2023, I completed and signed another parent and guardian waiver for my son Rain Fields, he also signed the form and had his photo taken at the Planet Fitness on Chapman Hwy in Knoxville, Tennessee.
The entire time, I had this membership for my son he has been check in denied.
I called my home planet fitness, 4 times and was told this problem and issue can’t be fixed.
Since day one, my son has had issue and problems check in with his black card membership.
I would like for Planet Fitness incorporation, to refund all my membership fees
on March 11,2023 and March 17, 2023.
And cancel my black card membership effective today, April 3, 2023.

On Monday April 10, 2023, the GM at the Planet Fitness on western Ave in Knoxville, Tennesse my home gym location.
She apologized for my son inconvenience and gave waved his black card membership fees and his annal membership fees as well.
My complaint issue with Planet Fitness gym membership was resolve by my "home gym general manager".
I never did got a response about my complaint from Planet Fitness headquarters as of today.

Hi there, I work at a Planet Fitness location in Vancouver, WA and hope I can help. So the alert in red that says "Check Agreement Status Queu" means that your son's account has literally been sitting in limbo within the system. It will stay that way until they approve it within the system. All they have to do is go into the "Agreements" area of their system and find your son's account and push it through. Now in my location if someone's agreement is sitting in limbo like that it won't charge the account because it's technically not a working membership yet, if that makes sense. So you shouldn't have been charged yet but that's if it has any similarity to mine which it's looking like it does. A few more things, his account says remote check in because whatever location you went to other than the one you signed up at isn't within the franchise that you had initially signed up with and you can only get this issue fixed at the initial location you signed up at. That was a mouthful lol. The last thing that is a little concerning to me is how you were asked to sign another waiver and another picture taken. This would only occur if you were a black card member and you brought him in as your guest or if another new membership was created for him and if it's the latter you will def want to cancel one of these memberships because there is zero reason to have two separate black card memberships. I hope I explained that well enough for you to be able to rectify the issue either in whole or somewhat. Sorry it was so long, just wanted to be thorough. Have a good day!

This facility is in dire need of help,
1. Facility is disgustingly dirty. Showers have mold, hair and debris in them.
2. Facility floor and equipment hasn't been cleaned for months, huge piles of dust/dirt in corners of facility and on and around equipment
3. Facility will randomly and FREQUENTLY NOT be open during posted hours because overnight staff doesn't show up and the manager there is worthless, he should be covering for this or doing his part to update the facebook page or the phone service so us customers are waiting over an hour in the parking lot for the center to open. This is happening at least 2 to 3 times a week.
4. Facility needs major maintenance inside, trim is off the wall in several spots and walls are damaged.
5. The employees, at least when we witness them, are constantly on their phones at the desk instead of attending the needs of the club, paper towels will be out, sanitizer out and all they do is play on their phones.
I would post pics but since we can't do that in the locker rooms I can't post them here.
Desired outcome: clean up this facility, it is a mess and in deteriorating condition, at this point of someone called the health department I guarantee you this facility will NOT pass inspection.
Exercise Equipment (hip adduction machine) 'temporarily' out of service for 4.5 weeks and counting.
The Planet Fitness Club in Gaithersburg, MD underwent a renovation in December 2022. Prior to the renovation, there were 2 hip abductor machines and 2 hip adductor machines and both received significant use. After the renovation, one set (2) of these machines was removed. It was explained by many customers to local management how stupid this idea was. Local management blamed the decision on "PF Corporate" and threw up their hands.
About 4.5 weeks ago, one of those machines (the hip adductor machine) broke and now has been out of service for at least 4.5 weeks. The regional manager was contacted about this and was less than useless. He too blamed "PF Corporate", whined a lot, and then did absolutely nothing.
Desired outcome: Fix or replace the hip adductor machine immediately.
Television channel provisioning
I have been a black card member of Planet Fitness ever since they opened in my hometown of Chattanooga, TN. I have always thoroughly enjoyed it and never had any complaints. Today (3/29/23) I noticed they had removed the FOX News channel and it has always been there. When I asked the staff they stated they did not know anything about it. I also noticed MSNBC and CNN were still there. I thought this variety always gave patrons a choice, but now that is gone.
I would like Planet Fitness to provide a plausible reason behind this decision. Stating that it was a "management decision" WILL NOT be acceptable. If this is the rationale, it should have been applied to ALL of the news stations and take them all down. For PF to be proudly claim they are the "judgement free zone", this is as judemental as it gets.
I do not want to leave PF after all these years because I truly enjoy it. But if this turns out to be a political decision, I will find another gym. There are many competitive choices available in Chattanooga now.

I was a member of the Port Jefferson Station NY Planet Fitness. I cancelled my membership 08/21. I was billed after my cancellation to 03/23. When I contacted the club after 2 conversations, they initially said they had stopped billing 08/21 they realized there were 2 people with the same name and then claimed I had only paused my membership, which is not true. Being charged while I cant use the service. I have the medical documentation as they requested to email them but they claim i have to come in person. I have no way of getting there because of hospitalization. Yet they still want to withdraw from my account and not give me credit for the weeks I havent been able to use service. from 3/9/2022- present

I have $10th, in fitness access thru my Health care plan. On three occasions in trying to get a membership via my Health plan, I made contact with three staffers, all of whom could have simply stated, "We do not accept Ins. Plans for membership."
Yet, in my first encounter, some guy who communicated with a robot-like tendency in response to my questions, I guess his way of being professional over the phone, told me a manager was not available, and took my info. so that once the manager got in, he would contact me. NO RETURN CALL/RESPONSE! I waited a week for a response.
The second week, I encountered a caucasian female, who told me I needed a code. Went home, and contacted my ins. co., who stated, I did not need a code. Finally, the 3rd time, I encountered Carla, Mngr., who took my info. again, NO/RESPONSE. three weeks passed, and I contact Corp/headquarters whom Carla/Mngr. said didn't exist, only to find out, they do not accept Ins. Plans. The staff is so composed, yet exempt from knowing such pertinent information.
I probably would have just purchased a membership on the spot, had they known their company DOES NOT ACCEPT INS. PLANS~~THEY WANT CASH! I'll go elsewhere.

I've been a member of planet fitness since early 2021, however I work disaster relief and travel a lot for work. I started my membership in Sulphur Louisiana which has been active ever since. I traveled to Charlottesville VA in January 2022 for work so I started using the planet fitness around February. For some reason when I went there they said my account wasn't active and I'd need to sign up again so I did not realizing I would now be charged at both locations. Assuming there was a glitch or some kind of error on there part I have now been charged 2 monthly fees from 2 different locations every month for a little over a year now. I called the location I first signed up with who verified my account was definitely active and they aren't sure why I was made to sign up again. I've reached out to the VA location who has no intentions of refunding my money even though I should have never had to sign up again. I need advice on how to handle this
Registration for minor
How is it that I had to have a birth certificate for my child for verification when he has an enhanced MI state ID. All was needed was the ID and me signing consent. The Woodhaven MI gym I feel was playing games because I inquired about the new process of having a guest because I hadn’t been there in over two years. I went on March 26, 2023 around 2:30pm. I was initially told I just had to send a link to my son. Then they switched personnel at the desk and the girl had a smirk on her face and told me that I needed a birth certificate to prove he was my son. I asked what if I was just a legal guardian and was advised it had to be the parent only. So I go on the website tonight because that info they provided kept nagging me, to see that it doesn’t require a birth certificate. Because how can a legal guardian name be on the birth certificate if they are not the parent. Y’all have some childish a$$ unprofessional employees working there. There are cameras and I have provided you with the approx. time I was there for you to do a further review.
Desired outcome: Them asking for a birth certificate makes me think they just wanted to get my social security number off of it and do lords know what. That is sensitive information and I don’t know if any other gym that asks for that information
Terms of membership
I signed up with the black card membership and it includes a guest. I have been going for a few weeks with my guest using all the extra services like massage chairs with my guest. I go today and was told that my guest isnt allowed to use the massage chairs? Ok I said but the employee that signed us up said absolutely go ahead.
Your advertisement there clearly states all the details of what is included in the black card membership below was on the table at biz. It does not state specifically what the guest can and cannot use.? I have a picture of it.
When I questioned this to the local Manager Lola she said its in your contract that anything the employee says is cancelled the minute I signed the contract. Really? Lola said I will let you 2 use it this time but not in the future. Well, ok so we go to use the chairs and they are broken...they were broken also when I first joined? If I have black card membership and the chairs are always broken or out of service? And I cant have my guest use them now what?

Certified fitness instructor in moses lake
I joined Planet Fitness in Moses Lake a couple of months ago. I love your facilities and I was really excited to work with a certified fitness instructor but was disappointed to hear you do not have a certified fitness instructor for your gym in Moses Lake.
Do you plan to get a certified fitness instructor soon? If not, have you considered developing and training from within? I go to your gym several times a week and I noticed one of your employees (Anita) is highly knowledgeable about gym equipment and very personable. In fact, Anita has trained for bodybuilding competitions.
I spoke to Anita and asked her what was holding her back from being a trainer and she said it was the cost.
I would like to take the opportunity and ask you to invest in Anita as a certified fitness instructor for your Planet Fitness program in Moses Lake.
This would be a win/win in the development and growth of your employee and much-needed service for newbies.

The facility is nasty; no paper towels, the toilet at first opening feces in commode, floors not mopped and clean with trash from the day before, all of these conditions have occurred in the last two months at this facility 6231 Perimeter Dr#217 Chattanooga TN 37421. I really can’t wait until my 65th birthday I am gonna join the YMCA. No one cares about cleanliness anymore at this facility the standards have fallen way below sanitary!

I’ve been thinking of writing a complaint for some time now in regards to an employee of Planet Fitness. Her name is Sarah and she works the late night shift. She on numerous occasions seems to be “odd”. I’ve observed her doing strange things. An example would be sitting in the bathroom stall for a very long duration continuously flushing the toilet when the showers were running. Another incident she was outside on the bench and was spitting up on the ground and most recently she started to vacuums and she will vacume the same spot for some time. She almost tripped me on the treadmill when the extension cord was wrapping around my feet. I’m sorry to say this but it almost felt bad if she did that intentionally. I am more than happy to speak to anyone in regards to this if you have questions.

Broken equipment not being repaired or replaced. This place is falling apart. Machines have been out of order for months at a time and there is no point at which they seem to come back in service. Used pads on equipment that aren’t repaired become the perfect breeding grounds for staph. Handles cracked and broken in tanning beds. Pos system broken or down and cannot purchase products. Equipment removed altogether and not replaced.
